之前仔细研究了需要的材料。发现纽约总领事馆和华盛顿大使馆的规定还不一样，需要
的材料也不一样，甚至申请表(H-1)都不完全一样。由于我家离纽约有7小时车程，为了
以防万一，我按照华盛顿和纽约要求的并集准备了以下材料（我一个人去，LP没去）：
1.填好的纽约领事馆的《护照/旅行证/海员证申请表》(H-1)一份；
2.照片：中规和美规各准备了8张；
3.儿子出生证原件和复印件；
4.儿子美国护照原件和复印件；
5.我的护照原件和复印件；
6.LP 护照原件和复印件；
7.我和LP均未加入外国国籍和拿绿卡的声明书；
8.LP同意儿子回国的声明和委托我一个人办理的委托书；
9.证明我和LP在美合法居留的移民局文件（I-797C H1B approve notice）。
以上所有复印件准备好后，按顺序排好，再打印了一张 document checklist 作为
cover，用曲别针别好，方便办事人员。
赴纽约前两天打电话(212-244-9498)到领馆询问，确认了以下事项：
