发帖数: 291 | 2 没想到啊没想到，去年才从Shelter领回的Aco小正太，现在才8个多月，前两天医生检
查后，说他心脏跳的声音很大，随即拍了3张X光片，发现心脏偏大。
最近Aco间或会咳嗽，所以带他去看医生，本希望一切安好，却没想到医生怀疑是他有
心脏病。真是晴天霹雳！医生说要带他去另一家心脏病医师那边做进行进一步检查，以
确诊是否因为他的心脏先天有问题，或者咳嗽是因为心脏问题引起。
他睡觉时呼吸频率是一分钟40次左右，医生说这是正常值的上限。呼吸频率快，也可能
是由于心脏问题引起。
他平时都活蹦乱跳的，丝毫没有任何异状，我一直觉得他是一个健康又皮实的美短小猫
。但医生说如果是心脏病，会随时有生命危险。Aco可能是看着正常，但身体内部机能
已有隐患。挣扎了很久，还是决定破财去给他做昂贵的心脏检查。
我不能把Aco送回Shelter或者送给其他人，他颈上的芯片里已经有我的名字。现在每次
看到活泼可爱的他，心里就有一阵隐痛。
真羡慕版上健康的猫们！
不知大家是否遇到类似情况？有心脏病隐患的猫，怎么让他长久的生活下去？ |

发帖数: 5094 | 9 我还看了一篇文章，后来找不到了。分析了CMS的数据，总共节省的钱里面，2/3是MGH,
BWH,BIDMC这三个医院来的。众所周知Harvard这老三家看病花费高，属于地主家把本来
该扔掉的剩饭,热热重新吃就能节约下来的。其他医院的数据，CMS根本不肯发布，可想
而知有多糟糕。而且我们这里，是靠拼命兼并小医院，扩大病人pool，才能把总花费省
下来。ACO基本就是个形象工程，就看政府什么时候承认错误了。 |

quality measure基本上还是血压，血糖，戒烟，心梗，心衰老五篇
另外就是追踪每个医生的处方和大的影像学检查，和同行比是不是总开CT,MRI,PET.能
开generic就不开brand name.
我们科基本没啥大动作。就记得直接开generic,少开MRI,CT,PET就行。马照跑，舞照跳
。我开始看ACO吹得山响说要搞quality measure的时候就奇怪，神经科ALS,CMT这些怎
么搞quality measure，难道比谁死得早？现在看就是多交点保险，多上点税，洗洗早

首先，这场战争是由谁来主导？在很多媒体报道中，法国对设立禁飞区，实施对利比亚当局的军事打击是最起劲的。考虑到法国在北非地区的某些传统利益，一般认为法国成了这场战争的主力军。

而言不那么在意时间轴。。。也就是 USAMO 可能更关心 space-pattern
（或者说不太区分 space vs time，更多的把 time 当成跟 space 差不
多的属性来看待），而不是像 USAPhO/USACO 那样关心 space-time-pattern。
或者说对于这道罗素理发师悖论题，不管做对做错，做题常常一定程度的
看到自己的潜意识。。。APhO / ACO 我觉得更多的在潜意识里把 time
看成 real number, linear-ordered 所以对应于与 causality (the
arrow of time), 同时还 measurable like geometric objects。。。
但是 unless synchronize time and "tick" time (严格意义上是
notation of the time)，time 本身不是那么 “countable” 的。。。

We just had a hospital retreat and had a speaker talked about ACO. I am
also aware there are some articles talking about ACO replacing insurance
companies. NYT had an article recently predicting 10 years later insurence
company will be gone.
My gut feeling is this thing is not going to work. This is exactly what
managed care is. Medical communism is coming to America. |

ACO的创建人是一个心脏外科大夫，后来不干了。O8care出来后立刻成立了ACO，在这个
”公司“里，俩是搞经济的，七八个搞数据分析的，全部都是phd出身。作为一个个体
，也就是小民俺认为，永远都是得“跟着政策走，紧拉着payment的手”，这样才能脚
步越来越轻越来越快活。
不是跟你较真找茬，我该感谢你，这两天正为一个marketing的策划苦闷，以前不知道

过程。Claim denial大多都是系统生成的，根本不需要人工确认。保险公司也会雇
vendor去调查一些claim看看能不能recover。现在几乎不存在保险公司少付医院的情况
，唯一可能是billing信息不全。大多数情况下都是overpaid,然后再recover。大公司
每年recover上百个million。
lz说保险公司的医生审查账单只能体现出他的无知。
