年，之后懒散了一段，终于开始做题，边做题边过FA，后期狠下心撇开孩子，高强度学
习感觉收获很大，本定八月考，因各种原因又推迟到十月底，终于考完收获成绩，感谢
学友和家人，达到预定目标。

初时过kaplan notes似懂非懂，只记住一小部分，直到开始做题才逐渐把知识串联起来
连成轮廓，UW题库真的很好，讲解详尽，条理分明，一个个知识点梳拢开来，头一遍做
的很慢，正确率只有60%左右，我做了三遍，到第三遍就很快了，一天能做六七个block
，正确率也上升到90%多，NBME考前三个多月开始做，Form2只有460分，打击颇大，之
后每天做UW，晚上跟着小组过FA，成绩逐渐提高，考前十天Form6 570分，CD是约了一
个practice session在考场做的，87%。感觉practice session 还是很有用的，可以提

患者44岁，男，6年前因为肝癌做了肝移植 （符合米兰标准），之后无灾无病好几年。
去年2016年初发现轻微贫血，血色素12，因为当时在服小剂量阿司匹林，于是停服，看
看有没好转。托托拉拉到了年底，还是贫血，血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤，内镜医生直觉是GIST。随后CT，但CT却没有异常。4月初第二次胃镜
，GIST依然还在，取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

搅蛮缠的病人。
昨天看了一个新病人的referral，说三十多岁的男病人，testosterone 升高。我看过
不少女病人testosterone升高，或者男病人testosterone降低，所以这个病例我格外留
意了一下。
看检查报告，那个病人的testosterone level在男性应该是正常的，但是数值边上写了
High。我觉得很奇怪。然后仔细看，那个实验室报告上病人的性别打成了female，所以
用的reference range是女性的，所以就报告是高的。PCP看到数值高，就大笔一挥，
endo referral.
我让他们重复了testosterone，把性别纠正过来，结果就是正常的。

this patient. I cannot really see the point of doing CT in this case, if MRI
was also requested later on.
I guess the renal biopsy is a must before consideration of nephrectomy?

other differentials are probably not very likely...

因为记忆中自己没有受过严重的内伤，顶多是发烧咳嗽之类的
如果下一步PET scan还是不能确定，要做biopsy的话，会很危险吗？
如果不知道这个东西是什么，冒冒然去戳它感觉挺可怕的。。。

PET scan做完了，那个mass没有亮
肿瘤科的医生说99%应该不是恶性的，至于是什么，还是不知道，连到哪里也不知道
怀疑有可能是长在adrenal gland上的瘤，也有可能是cyst，也有可能是gastric
diverticulum。。。
肿瘤科的医生认为，因为我能感觉得到这个mass并造成不适感，他建议切除，并把我
refer到了一个外科医生那里
明天就要去见外科医生了，心中还是比较忐忑的，毕竟还是不知道这个东西是什么，而
且对于接下来要怎么做，也没有概念。
大家能给点建议吗？我自己想，先要搞清楚这个东西是什么，连在哪里，etc
可能要先开个口子在体内成像？然后再决定怎么切？还是直接切呢？
这个东西的位置感觉挺隐蔽的，可以做微创手术吗？还是要开大些？
这种手术危险吗？死亡事故的概率大吗？如果正常的话，要休息多久呀？

我一个最好的朋友的母亲得了Adrenal Cancer，正在做化疗
她的医生想用一种药 Lysodren 500MG 100CT。可是中国各个医院药店都没有货了。
我查过这边有卖，可是是prescription的药，需要有处方。
如果有朋友有医生执照能开处方药的话，能联系我一下么？您可以和国内医院的医生连
线，并发给你病历，然后麻烦您开处方，这样我可以帮我朋友在这里买到药救命。
而且这样药同样在宠物药店也可以买到（有一种狗得的病也要用这种药）我和制药厂打
过电话确认了，人用和狗用的药是同样的，没有不同的型号和包装。所以如果有朋友有
兽医执照的，也可以帮我开兽医的处方。我可以提供狗狗的信息。
