Where can I get the AECR application form? Do I need to ask schools to send additional trancripts directly to NASBA?

Academic Evaluation for Certification Report (AECR)
下面注明：
A completed AECR is required from all candidates as evidence that you are in
compliance with the Massachusetts educational
requirement of 150 hours per 252 CMR 2.07(2)(a). If you do not meet this
educational requirement, you do not qualify for a
non-reporting license.
AECR is not required if as of November 2002 you were approved for the 150
hour requirement with your First Time Massachusetts Exam Application.

核可以sit for the exam的复印件过去，结果人家很快email说不用AECR，还把申请费
refund给我了。于是我就直接寄申请表申请license了。 |

我是2009年8月份，在Delaware州考完的CPA，之后申请这个州的certificate。但是他
们要求certificate和license是一起申请，要求我提供一年工作经验。我那个时候还没
有一年足够的经验，就没有提供。结果这个case现在变成“abandonment status".
我只有半年的public accounting 经验，所以就想转到MA申请Non reporting license
。 按照程序，还要交100元申请AECR。我刚刚信用卡付了款，结果才发现3年成绩就作

我在申请麻省的non-reporting license，州里要求提供NASBA审核的ACADEMIC
EVALUATION FOR CERTIFICATION REPORT (AECR). 我完成了NASBA的网上申请，也邮寄
了纸质的硕士学位成绩单。
随后NASBA寄给我一封电邮，内容让我很困惑，不知道自己下一步该干什么，请问大家
能说一说NASBA在这封信里是要我做什么啊？ 电邮内容如下：
“Your NASBA Licensing Application has been closed. Your application and
supporting documentation will be kept on file at NASBA for 1 year in the
event you should reapply. If you should have any questions or need
