对于基因敲除（gene knockout）小鼠大家并不陌生，比如我们想要研究一个基因
在体内的功能，我们就可以把这个基因从小鼠体内敲除掉。而有的时候我们除了关心一
个基因的功能外，还想知道表达这个基因的细胞在体内是干嘛的。比如，我们想知道B
细胞是干什么的，胰岛细胞是干什么的？我们可以从小鼠体内把这些细胞清除掉（也叫
细胞剔除，cell depletion），一看不能产生新抗体了，或是胰岛素没了，血糖升高器
官衰竭了，我们就可以恍然大悟说：“你看，胰岛细胞是产生胰岛素的，B细胞是产生
抗体的”。然后就可以发文章到CNS（Cell, Nature, Science）了。当然这些都被别人
发现过了，是我们没有赶上好时代呀。
那怎样才能在体内把一群细胞清除掉呢？方法可能很多，但也真不多。这里我讲一

coomassie染色， 发现这个蛋白不在已知的pI位置上，并且条带很宽。同时用作对照的
albumin pI比其正常值（pH 4.7）低。我检查了缓冲液浓度和pH，都是正常的。 请问
各位牛牛，这个是什么原因造成的？怎么解决？急啊

推广的醣类计算饮食，发现，两种皆能够有效控制血糖，然而全素饮食对血糖的降低比
糖类计算来的更加有效。作者 Barnard et al. 表示，全素饮食有效控制血糖主要原因
可能来自较低的热量及脂肪，在此情况下，全素试验组的体重降低较多，可能因此增加
胰岛素敏感度。 此外，高脂饮食，在之前的研究发现，可能造成肌肉细胞及肝脏细胞
囤积较多的脂肪，导致细胞对胰岛素不敏感! 而胰岛素不敏感是导致第二型糖尿病重

糊涂，她血压总是偏低，用midodrine 10mg tid，护士还page我说低，都50多了，我赶
紧给了100ml albumin (ESRD on HD)。等跑过去一看病人很舒服，自己量了一下血压其
实是80多。你们dopamine drip在病房用吗?护士很强大。至于用 heparin drip，估计
就是像逆风MM说的，如果要是有可能做啥手术的，heparin drip比lovenox更容易控制

该患者诊断： Nephrotic syndrome 。 依据：1） proteinuria 2）no hematuria.
另外容易形成静脉血栓（DVT）. PT has more cold is related to low albumin.
So B can be excluded first.
C(RPGN) and D(GOOD pasture) belong to Nephritic syndrome, should have
hematuria, so could be excluded ,too.
Answer A is Minimal Change Disease, a disease of nephrotic syndrome ,
but
usually affects children (peak incidence at 2–3 years of age).
So my answer is D(Membranoproliferative glomerulonephritis). |

结束后,6月28日小便出现尿血,按常规止血方法均告无效。后上海专家用介入法,在大腿
根部微创一个米粒大的口子,用动脉造影的方法去找出血点,发现膀胱壁上有一片渗血处
,用了血管收缩剂,效果甚微。姑姑现住在医院里，每天靠输血（300-450ml）和人血白
蛋白（Human Albumin）维持。由于失血多，不能下床行走，只能躺在床上大小便，浮

if we suspect of bleeding (decreased sbp, cvp and even co, and massive
drainage from jp bulbs), stat cxr and h&h will help identify the real reason.
give albumin fast to maintain intravascular volume when both cvp and sbp are
low. sometimes, prbc transfusion is needed right away. |

given high concentration albumin and lasix at the same time to increase IV
volume and deplete interstitial volume. It's not guideline recommendation,
but it worked in some pts. It is fairly expensive too. Need CVP and PA
monitoring at the same time.

A1: 药物吸收增加(cmax, AUC),albumin降低（影响protein binding的药物的自由浓度
），最重要的变化之一是代谢的P450变化：3A4, 2D6活动增加, 1A2的活动降低。最后
是Clcr会增加，鉴于肾脏供血增加，GFR也变大。
Q2: 现在FDA对于孕期药物风险分类有什么态度？
A2: 逐渐淡化，因为简单的A,B,C,D,X分类的信息量不足，产生许多误导。C类药物最不
明确。FDA对于新药已经强制要求有fetal risk和lactation risk summary。淡入的是
每种药独立的数据和风险归类。
Q3: 孕期最重要的几项维生素补充有哪些?
A3:
1）铁元素是生产红细胞的必须，每天补充含量为30-65mg。鉴于前三月的孕吐严重，可
以考虑前三月过后开始补充。
2）钙元素为骨骼发育的必须，每天补充1000-1200mg钙元素。
3）叶酸则比较特殊。最好的情况是怀孕前1个月开始每日摄入0.4mg（ACOG&AAP甚至推
