后面的range(0-62, 28-100, 5-55)是正常范围。
ALK. PHOSPHATASE 84 0-62
ALT (SGPT) 161 28-100U/L
AST (SGOT) 58 5-55U/L
从检查结果看明星的肝功能问题。大夫也说不清是怎么回事。但是看在他能吃
罐头的份上，先观察两天。另外我知道一个很好的大夫，就是远了点。打算这个
星期带上尼尼去这个好大夫那里，一起带上从现在大夫这里得到的血液检查，
X光片子。

另一个vet Dr.B（我不喜欢那个vet)，今天是Dr. G, 是kaka一直见的vet.G.他说几项
肝指标偏高，可能是infection引起的secondary,也可能是肝炎之类，可能cancer但卡
卡这么年轻先考虑排除别的。 Dr.B让查T4，问Dr.G为啥，Dr.G也不明白打电话问Dr.B
,说是胆固醇高，要查T4排除Hypothoridism,我问拉肚子呕吐什么关系？？Dr.G也不
convince该查那个。。最后他说，回去吃药观察，如果继续呕吐拉肚子，回来拍片子，
再查血，或者fecal culture排除salmonella?
感觉vet意思是，如果吞了异物，呕吐应该更频繁，他吐得频率更像是功能方面的。血
检结果贴在楼下了，有懂的帮忙看下吧，多谢了。AST (SGOT), ALT(SGPT),Alk
Phosphatase, Cholesterol偏高。
今天早上吃了鸡肉拌饭，胃口也一般，目前还没吐，便便还soft一滩。

能说得过去，所以要求很高的浓度和效率。也有可能是自连，就是在两个酶只有一个酶
成功切了，而另外一个没有切的情况下，肯定自连。因为需要连接的效率，所以ALK之
类的处理被排除。也不想分布酶切，因为首先有共用buffer，其次我不想多纯化一次损
失我的vector.按诸位的意见，我打算降低单个酶切反应dna的量试试看。另外我这两个
酶切位点相距12bp，我想不会相互干扰。 这12bp中还有另外一个酶切位点，也是同样
的工作buffer，我想先用双酶切后，再加点这个酶，争取把那些漏网之鱼逮到干掉。
诸位有好主意，请不吝赐教！谢谢 |

First, I do review both grant and paper but nothing to be proud of. In the
begining, you will be excited to do it but then you will be sick of doing it
. Also you spend your own time to do it and it became a huge burden. You
probably know how hard to condense 20 page information to 12 pages for R01,
but is much harder to just read those things, especially so many of them.
As for the personlized medicine, diagnosis or therapy, it is the trend and

,看看有没有EGFR, KRAS, ALK mutation.有些基因型一些比较贵药效果会比较好,比
如tarceva之类的.如果没有办法测序,目前估计docetaxel, cisplatin+pemetrexed
(Altima)好像用得比较多.不直接做临床,只知道这点. |

现在说用高通量测序做癌症的都是忽悠。
暂时用不上不代表没前途，和药物开发一样，现在是硬件软件数据平台积累阶段。
第二点也不完全是。还说EGFR，同样是sensitizing mutation, 如果用PCR鉴定，确实
应该是使用的。但有些人就是有原发或激发耐药，结合omics还是帮助诊断。
如原发耐药 or 继发的c-MET，ALK突变。 |

是如果有就可以用crizotinib，效果很好。（Kwak et al. Nemj 2010)你可以google一

试验
对美国实习生对国内医疗水平的认识深表遗憾，肿瘤内科应该是最没有技术含量的工作
了，国内都快烂大街了，巴不得街道医院都整个肿瘤科，全世界都用一样的指南、甚至
是一样的药物，照着开药就行了，会处理点并发症就可以了！
crizotinib 只针对ALK基因融合突变的患者，非小细胞肺癌里大约5%，小细胞的更少了
，如果做了基因检测存在突变自然可以用，但是概率太低。 |

患者一月初自己摸到右腰部皮下有一结节，不痛，于1月20日将结节切除，1月21日病检
为转移性低分化癌，说是恶性程度比较高。
1月22日做PET－CT发现右上肺有包块，应该为恶性，大约4cm。其他器官未发现转移。
1月23日晚上咳痰发现有点带粉红色。
1月24日入院，26日做了纤支镜，取了活检，但是由于又拿去做基因检查，所以结果还
没出来，当时医生涂片后跟我说是非小细胞腺癌。
朋友自己查了很多相关文献，这种癌症貌似预后不太好，生存期也不是很长。
他们的问题就是：
检查只做了ALK和EGFR，没有突变，还可以用靶向药么？还是需要做其他的基因检查？

最近科室里面掀起了一阵CCRN潮，科室还给大家一套AACN的复习材料给大家轮流使用。
AACN 有一盘CD 和三套模拟训练题， 我只看了CD，觉得非常有用的，每个系统把考点
总结了一下，很好的加强了理论知识。
我还用了pass CCRN CD 的题库，没有全部做完，cardio 和 pulm 实在太多了，大概各
做了70题，
其他category题目少的基本都做完了，每次无论对的错的都看解释加做笔记，虽然笔记
后来一遍都没看过 ：（
总共复习了两个星期，其中全职上班,不上班的日子每天复习8小时，上班的日子完全不
看书。
实战感觉和 pass ccrn 接近或者更简单一些，基本一分钟一题，题干也不长，磨棱两
可的做bookmark，做完全部150题 然后回头琢磨那些磨棱两可的，根本不需要三小时。
以下理论知识基本在考题中都有用到：
CVP, PAOP, PAP, CO, CI 这些正常范围一定要知道，怎么区分LVHF 和 RVHF，fluid
status,
preload, afterload,
