Allscripts Healthcare Solutions Inc. terminated its chairman, a move that
prompted the resignation of three board members in protest and contributed
to a 40% plunge in its stock.
The Chicago company, a provider of electronic health-care records, also said
its chief financial officer was leaving to pursue another opportunity. The
news came as Allscripts reported a 54% drop in first-quarter earnings and
lowered its guidance for the year.

dictations and notes ready when the visits are done.
But allscript has to have someone type in the phone messages from the
patient. Epic sounds like skipping this step and directly communicating with
physicians through the web.
吸取个人的经验教训，特别是怎么bill，怎么screen 病人之类的，老医生都很有一套。
几个常见病去都各有一个模板，根据模板问问题既高效又省事，而且最要紧的是不会遗
漏，过去史家族史药物之类的都有自动link，vitals每次都自动更新，还可以同时看过
去几次的vitals, labs。另外用模板事先根据medicare billing的要求设计好ROS,

NYHQ做OB还有一些问题，所以希望写下自己的经历，对后来人有所帮助。
首先非常感谢ACAP的帮助，让我们在NYHQ能一直有机会。我来到NYHQ医院做了一个
月的OB. 虽然知道这个机会来之不易而且还有很多XDJM在报名排队，我在来之前还是有
些犹豫。因为可以在我所在的城市里的教学医院做OB，为什么要离开家人和工作，来到
一个社区医院做呢？其实来到第一天的见闻就打消了我所有的顾虑，一个月确实挺值的
，只是没有机会再做长一些时间。
NYHQ位于法拉盛区（Flushing），医院大厅标注着英文、中文、韩文、俄文等等，
足可以看出医院的多元化（diversity）。第一天先参加orientation和infectious
control training， 然后去街对面GME办公室递交application form等paperwork，再
回到医院办理ID卡，到五楼找chief resident报道。一批observer一般有四人， 两周

right will be greatly limited but RN will be ok.
Last time when I questioned about E-mail communication, I was told to wait
for Epic. Now I get used to current system, you deliver me a bad news. :)
Will need to find a way to get around when epic is implemented.
So do you charge the patient when you have to return the massage? Now I
think putting on a charge could curb the patients abusing the system.

发帖数: 9638 | 8 我最近刚刚放弃allscript，转成Epic了， 据说是当今最好的EMR.
好处不用多说，我想说说我的担忧：
这个EMR从某种程度上是一个智能体，它潜移默化地影响你的每一个看病步骤，从病史
，到查体，再到诊断，然后就是治疗。 你如果有自己的想法，这个智能体不接受，你
就只能放弃。当然这个系统同时防止了你的出错。但不得不承认，它对每一个医生的影
响是深远的。
举一个例子：
头疼是一个常见症状，大多数情况，你不知道什么病因，所以可能有几个假设，比方说
migraine， cluster headache， trigeminal neuralgia，etc。传统的病历我们会把
这些可能性写下来， 然后说出自己对每个可能性的想法， 具体相应的方案是什么。
但在EMR里，你必须给出诊断。 你可以说headache，鉴别诊断你就需要另外写下来，而
写那些notes不是EMR所鼓励的，在billing也得不到任何credit。 一个好的H/P或
consult就是要看诊断分析，这是病历的灵魂， 在EMR却大大的弱化了。
