标 题: 高中好友突患恶性肿瘤gland alveolar rhabdomyosarcoma，请各位

我决定同时把这个转到一些人起比较高的热门版面，希望各位斑竹谅解，手下留情！
前天上MSN从一高中同学那里得知，一位同学突患恶性肿瘤，病情相当严重，因长沙医
疗水平有限，请求大家尽力帮忙想办法，挽救他的生命。
基本情况如下：他于2007年1月感到颈部不适，医生初判为颈椎骨质增生，随后疼痛加
重，2月26日入住湘雅附二医院。经核磁共振检查，发现颈部绕第二节脊柱有一8*5*2cm
大肿瘤，在喉咙正后方，严重影响进食、讲话等基本功能，3月5日进行了肿瘤切除手术
。肿瘤已扩散，并且位于颈椎顶部，无法切除干净，手术结果不理想。3月14日病理切
片结果诊断为：分化差的恶性肿瘤，符合横纹肌肉瘤（腺泡状型）。湘雅附二医院专家
会诊后告知：肿瘤恶性程度相当高，已发现上述两种癌细胞，并已扩散，肿瘤位置差， |

久的问题。
以前看中世纪的电影，里面英格兰的国王等在念比如arabic这种词的时候，中间的r有
时候会被发为
alveolar thrill,特别在着重的时候。后来我想到德语里面两种thrill都有，特别是贵
族，英德又
都是日耳曼语系，于是我想，是因为英国受德国影响？还是英国受苏格兰语影响？还是
苏格兰语，德
语，英语互相影响？在阿拉伯的劳伦斯里面英国人把arabic年成alabic的，只是似乎没
有alveolar
thrill之，那这是不是说明英语的变化是alveolar thrill的r演变到发l的音再到今日
的r（卷舌
as in ribbon）？那如果是的话，为什么今天的英语要变成词中间的r（比如Arabic）
要念成卷舌的
r而不是保持原来的alveolar thrill或者干脆念l？ |

是s音)了。。。。
随便找个linguistic department做phonetic transcription的人都能发现。
这点是可以用任何linguistic analysis的软件分析出来的，两个音的噪音谱不一样。这点不论是英国，美国，澳大利亚，只要是母语是英语的人，都有这个分别inter-dental和alveolar的contrast。
没有这个contrast,基本就不是母语。。
大多数中国人，没有这个native contrast－－－其实我明天回系里，都可以把她声音
ｌｏａｄ进去分析，你就会发现，她的（ｔｈ）音和（ｓ）音噪音谱是完全一样的。
当然，虽然有这个accent，但是不能磨灭ｍｍ英文很好的事实。这点小瑕疵也不算什么
。人家学得挺好的。
说英文有没有口音，也不是什么大不了的事情。有口音，恰恰证明了你的originality
，是值得自豪的事情，没啥好自卑的，或者好争论的。 |

, and it's basically a diagnosis by exclusion, the patient would need to
have a bronchioaveolar lavage and transbronchial biopsy to prove there is no
vasculitis or that sort, bascially the patient has alveolar hemorrhage that
could eventually lead to pulmonary fibrosis, the lavage would show
hemosiderin ladden macrophages, so patient often became iron deficient later
on, the cause for mortality would be acute bleed or |

周一早上去拔牙，之前医生也没有说什么可能会有的后果，因为LD在其它医生那里做过
相同的手术，也没什么担心就直接签了consent form.
手术貌似也很正常，反正我就是睡着了，半个小时后做完手术清醒后LD带回家。第二天
大部分脸部已经恢复正常了，可是lower teeth, lip&chin到现在都是麻的。打电话问
DR OFFICE一直说是正常，告诉我up to 5 days.可是我自己感觉没有一丝好转的迹象，
上网查后感觉可能是伤到 inferior alveolar nerve，没有其它好办法，只能等。网上
的人三年五年还没好的人都有。现在心情实在是很郁闷。其实这个智齿暂时还没影响到
其它的牙，也不疼，只是我的牙医说长歪了让我拔掉。
我把自己的经历写出来告诉大家，希望大家以后做类似的手术前要三思，看医生时一定
问清楚可能的后果。 |

我的情况和你差不多，根据你的描述，我觉得是医生拔牙过程中伤到了你的inferior
alveolar nerve。你可以再回头看下你的X光片，看看是不是有根神经离牙根很近。我
当时的神经正好在两个牙根之间，所以很容易被伤到。医生一般会复查，复查时告诉他
你的CONCERN，看他怎么说。我后来查过网站，一般好像是6周左右可以做nerve damage
test,来决定神经是不是permanently damaged。如果是的话，可以做个神经搭接手术
，但这个也不保证一定会成功。如果是temporarily injury, 可以开营养药，但说实话
我觉得这个就是一个心理作用，最终还是需要神经自己慢慢愈合。
你的痛苦我非常了解，当时我还要讲课，讲英文还要张大口，可是嘴根本不可能完全张
开，当时真是非常痛苦加郁闷。ANYWAY, 我差不多是六个月才完全好的。如果你自己有
感觉情况一天天在变好，那多半是临时的损伤。经常自己做个冷热敷，十分钟冷十分钟
热来刺激神经，外加按摩。祝MM好运啦。 |

is a moot point. there are way too many 2nd generation chinese who, in

which means there is still ventilation, but oxygen can't pass through the

doesn't help for secondary PNA. Patients need mechanical ventilation with
high PEEP to keep alveolae open.
