be easier than program B, as there are less floor months and unit months.
As of call schedule, there could be Q3call, Q4call, Q3 is definately busier
than Q4, that means you ahve to be on call every 3 days, you are working on
regular schedule on the two days in between. Also the frequency of long call
matters, that means overnight call, usually happened on weekends. now there

4 weeks electives, 4 weeks night float, 4 weeks ICU (only Monday to Friday
7am to 7pm), 4 weeks vacation, rest of them are floor rotation.
PGY2:
8 weeks ICU(12 hour shift and off on Saturday), 8 weeks night float, 4 weeks

1. PCP: Family Medicine Physician 和Internist 为主。两者的差别在于：FM看
内外妇儿，门诊为主，In-patient的训练很少。IM是inpatient加上门诊，inpatient的
训练是重点。
2. 成人病人看内科门诊就是看PCP。PCP决定，病人什么时候要住院，什么时候进
ICU，什么时候请会诊。比如COPD不一定每次都请会诊，我知道怎么处理就自己处理了
。 比如chest pain 病人，需不需要procedure，请心内会诊，PCP来管理病人。如果是
在医院，那就是 hospitalist来管。比如一个GI bleeding 病人被收入院，就是以
hospitalist为主管。

但是可以电话讲。就讲了自己的情况，护士当做传话人，医生说正常。整个过程不到5
分钟。
后来，收到了一张$400的医疗账单，说是referred ambulatory。问题是，我是这位医
生的病人，也没有任何refer，怎么会是referred ambulatory呢？请教知道情况的朋友

medications: Lortab 5 mg
然后还有一张写着Ambulatory staging pre-op orders: 就是一堆list，没有任何印
记，医生也就签了个名。没看到有写Anesthesia Record和Pre-anesthesia care
record啊。。 |

我跟老公去年7月搬到这里，12月28日去做annual physical check。老公的保险是100
％ coverage within network; 80％ coverage out of network, plus 500
deductible per person。
两个人的常规检查而已，2月收到账单，加起来要1100多。
先打电话给health insurance company，说是因为out of network了（500＋500＋20％
＝1100多）。但是我们去之前确定了，当场也确定了，是within network的，一番艰难
沟通，保险公司说Waltham Family Medicine bill错了，用的是"Newton Wellesley
Ambulatory Services"的名义——这个诊所在Newton Wellesley Hospital里面租的房

不过火警训练是每年都要的。
火警响了，第一就是关门，每个门都是防火防烟的。医院的人员配置不可能做到搬离病
人。因为火警一响，就不能用电梯。现在的病人，至少1/3是不能自己走的。很多都是
完全卧床的。我们一般是6个病人一个护士，8-12个病人一个护工，能照顾独立行走的
病人走就不错了。完全卧床的病人走防火通道那是不可能的，除非是FIREFIGHTER来背
。所以只能按POLICY来做。
当然大部分是假火警。但是真要是本楼层着火了，估计只能是照顾能行动的病人逃离，
抢救剩下的病人只能是消防员的工作了。
平常把一个不能行动的病人从CART移动到床上都要4个人。你怎么抢救。
除了关门，碰到突发的火灾，所有的医院POLICY都是R〉A〉C〉E

blood pressure monitor，是48小时内自动每30-45测一次，但国内几乎没见过。其次
是Home blood pressure monitor，自己买一个血压计在家测量，然后取平均值。不过
听你父亲多次测量的样子高血压需要长期长期服药的可能性比较大。新型和旧型血压计
准确性应该没区别
2. “总觉得西药毒副作用会比较大”是国人普遍存在的一种特别严重的误解，不客气
的说，我觉得和上世纪初照相机刚刚进入中国的时候国人有所畏惧有一拼了。常见用于
降血压的现代医学药物非常安全，而且高血压的治疗正是基于获益>风险的原则的原则
才去治疗的（简单来说你可以把给你父亲吃的西药当成保健品，延年益寿）。血压确实
可以通过生活方式注意来降低，比如每天尽可能多吃蔬菜水果，多锻炼，但是对于你父
亲这个年纪概念生活方式不一定现实，而且你父亲生活方式好像已经足够健康了（每天
都慢跑），而且有非常安全有效的治疗也不需要。
食疗和中药不仅不靠谱，中药导致严重肝肾损伤在国人中倒并不少见，安全性非常令人
堪忧

1. There is oral surgery department, only for teeth extraction, no other services.
2. You can get a physical at ASC ( ambulatory screening clinic ), it is first come first serve, average wait time is 5-6 hrs, clinic also opens on saturday but only for half day.
3. Drug is not free but it is very cheap though, couple bucks for 1 prescription.
4. For specialty referral, you need go to ASC first then get referral. |

Kaiser Southern California employs thousands of doctors. So I wouldn't be
surprised if Kaiser has the largest total # of complaints and medical errors

向attending负责的。我们这里有些attending从来不和intern说话的。
有些intern在他们本国作完了住院医训练，只要过了最初一段适应期，比起向我这样以
前从来没有做过临床可能还会好些。 |

Cholecystectomy is a minor surgery. In some medical centers of US, its an
ambulatory surgery, which means patient may go home at the same day of
surgery. I dont think it would increase the risk of rectal cancer. |

Primary care in IM - The Primary Care Track is designed for residents who
wish to prepare for the practice of Internal Medicine in the ambulatory
setting. The curriculum includes more outpatient rotations than the
categorical program and includes exposure to office-based dermatology,
otolaryngology, women's health, orthopedics, geriatrics, and preventive care
among others. |
