昨天，这些人折腾了一天，白天，晚上皮啦啦，也不知道谁把谁杀了，谁把谁带走了。
反正我先生和孩子被绑架了，就不知道是不是已经遇害了。我本来是打算睡觉的，但他
们头上，地下装电磁波攻击器，让我几乎窒息。虽然我没倒下，但其实他们在我身上扎
的毒针已经蔓延，我能感觉到我的呼吸有窒息的感觉。我想我对他们已经非常仁慈了，
但他们不依不饶地攻击我，欲置我于死地，让我非常生气，特别是肆意伤害我家里的人
，绑架我先生和孩子，而且有许许多多无辜的生命毁于他们之手，有必要把他们曝光。
当然我还没分析这里边是那几个人具体攻击我，伤害我的，但他们显然都是一伙的。鉴
于他们伤害我的头部，胸部 和胃部，我就把这几部分的医生都曝光，受害者慢慢去调
查，追查他们吧。

了就算了。
举个小例子。外院转来个病人，头疼，腰穿怀疑是SAH，CT head啥都没有，做了
cereberal angiogram，发现一侧vertebral artery disecting aneurysm。
看外院的angiogram，那个aneurysm离PICA挺近(想想wallenberg)，intervention
neuroradiologist有点不太想做coiling。病人这两天觉得头疼变好了，不如送回家观
察算了?
结果病人第二天早上说后背疼.attending的hypothesis是因为血向下流，所以头疼好了
，该成后背疼。重复腰穿还是继续出血。于是决定做coiling。可是aneurysm填上了，
病人整成了wallenberg怎么办？他们让病人在清醒的状态下做angiogram，用ballon做
测试，就在cath lab做neuro exam，确认没有问题后做的coiling。那个手术从早上一
直做到了下午.

血检结果是尿素水平有点高，could form crystals, so should continue with S/D followed by C/D。
还有就是Fluffy的血液里有点aneurysm的迹象，不过完全有可能是这次的crisis造成的。所以vet建议blood test in one week & urine test in three weeks。
疑惑的是，aneurysm验血能验出来？

一般5mm以上的aneurysm。 carotid dissection， 你就需要注明neck MRA

When I dictate a report, I always include the following:
Intracranial portion of internal carotids including petreous, clinoid and
siphon
Anterior cerebral arteries, A1 and A2
Acom and pcom, look carefully for aneurysms
Mcas, need to follow at least to trifurcation
Pcas, p1 and p2, if there is fetal origin of pcas
Basilar artery, vertebral, superior cerebellar arteries, pica and Aica.

说一下CEA吧，做这个手术的医生什么科的都有，美国医生做了一段时间后就有一个手
术并发症的比例，如果超过5%就别做了。 应该承认，医生之间还是有差别的，可以很
大，比方说1%对4%， 后者是4倍，但总的来说还是很低，所以没听说什么医生为此失业
的。美国医生标准化的训练对缩小医生之间的差距很重要。
做手术多水平就高？ 我还真不认同这个观点，我做了这么多年的腰穿，穿不出来还是
穿不出来，每年都有那么几例。
我感觉，一个好的residency training加上1-2年的独立工作，他们的水平就没什么太
大差别了。当然不会做的做再多也没用，当年北京最好的神经外科医院做aneurysm，做
一个死一个，做了那么多例也没个长进，现在估计好多了，美国做aneurysm有几个做死
的？
至于天赋，我想就是1%和4%的差别，做CEA的医生我常用的有两个，一个是certified
vascular surgeon， 另一个是普通外科医生，我感觉后一个并发症更少。
有意思的是我常用一个骨科医生做颈椎手术，他是退役的NBA player， 没球打了就去

1. i looked it up and 基底节区 it basal ganglia, assume she had significant
spontaneous intracranial bleed due to poorly controlled hypertension and the
bleed extended into ventricules, so she had ventriculostomy to relieve the
intracranial pressure, is that right? this would be much different than
subarachnoid bleed that needs to look for any aneurysms or vasculitis and
such,

is up to 1%. Most of them will be symptom free for the rest of the life. |

隔几分钟在上下转换,不是那种尖锐的痛,是那种揪住的感觉,后来就感觉上腹部不舒服,
感觉向上涌,犯酸,要吐又吐不出的那种.后来这疼痛一直持续了大概六七个小时就慢慢
好了.再后每隔几天发作过几次,现在不是很疼,但下腹部还是隐隐不舒服.两周前看了医
生,验血查了肝胆肾都正常,随后做了CT.CT结果如下,好象说是CBD肿大,肝部有点小东西
.听医生的建议约ERCP专家,但专家要一个月后才能见我.我现在拿不准是不是就等这个
专家,还是坚持家庭医生先做MRCP.麻烦专家指点下.多谢.
