关于HIV与细胞融合好象跟受体的识别是有着极大关系的。
HIV-1本身是有外壳蛋白包被的， 它的衣壳蛋白复合体在病毒入侵的
过程中是起关键作用的。 并且还可以诱使已经被侵然的细胞与相邻的
细胞发生融合这个现象就叫做syncytium formation(HIV真是狡猾呀！)
HIV-1衣壳蛋白复合体最初合成出来的前体就叫做gp160. 然后这个前体
经过修饰和剪切后变成两个亚主分，就是表面亚基gp120和过膜亚基gp41，
他们两个以非共价键相连。
这个gp120后来就会结合CD4和一个coreceptor， 这个coreceptor是一个
chemokin receptor家族的跨膜蛋白，经常在容易被侵然的细胞表面出没
比如T 细胞，和巨石细胞。然后穿膜的gp41就发生一些构想得改变，
开始介导细胞的融合过程。现在这个过程中等每个成员都变成了
antiviral cmpounds的耙子。

microRNA(miRNA)是真核生物体内一组内源性的 21—25nt 长的非编码蛋白质的短序列
RNA，它们能通过碱基配对的方式与生物体中的 mRNA 分子相结合来参与调节基因的表
达。自 2000 年至今，miRNA 的研究发展的速度极快并且影响的范围也非常的广泛，而
且分别在2002年和2003年度中miRNA入选了Science杂志年度十大科技突破。因此，
miRNA是 RNA 研究的又一个突破，被称为 RNA 的第二次革命。目前的研究已发现
miRNA 参与了很多重要的生命活动过程涵盖了发育、凋亡、代谢、人类疾病以及病毒侵
染等方面。在病毒侵染宿主和宿主抵抗病毒的过程中病毒编码的 miRNAs 和宿主编码的
miRNAs 均发挥了重要的作用。这无疑为病毒的侵染以及宿主抵抗病毒的侵染的研究提
供了新的思路和新的领域。因此，本文的主要目的在于阐述 miRNA 的一些基本概况以

啊antivirals，antifungals等等等等能用的全都用上（环境当然可以无菌，人体是绝
对不可能无菌无毒的），放他个一段时间（时间长短目前还不能确定。这期间还得不断
的把新生成的所有免疫细胞继续干光），那么，这之后，我估计HIV应该可以被成功清
除，如果病人还活着的话。 |

我承认innate immunity里面新蛋白也是有很多，但是重要性还是不一样的。
在MAVS之前，估计james chen在antiviral immunity领域的影响仅限于NFkB/ubiquitin
signaling吧。还有sting,几乎所有新发现的DNA sensor（除了TLR7/9)都是通过sting
signaling来调控下游抗病毒反应.
我不认识舒，仅从我学的领域来评价学术。
有时候作出一个重要的发现不需要太高的水平，但是第一个发现还是很有地位的。就比
如说发现GFP的下村修，水平也许不如Tsien,但是诺贝尔还是要和他分享的。
一点浅见，期盼抛砖引玉。 |

本人在博士期间主攻病毒学，用生化手段研究病毒的replication regulation, virus-
host interaction, antiviral therapy等。主要研究HCV，在JVI/PLoS Pathogen上各
有一篇一作/共一作的文章，同时也有合作研究Flu和一些黄病毒科的JEV, WNV等，发表
一些小文章在Archiv virol, BBRC, Gene, BBA-General等。现在做博后第二年，主要
研究先天免疫反应调控，涉及到病毒感染引起的宿主免疫反应方面。恳请有审稿机会而
又不需要继续攒review的牛人帮小女子推荐啦，谢谢！！ |

多个制药公司都有这方面的项目在做临床前评估，anti-PD-1或anti-PD-1L用于治疗CHB
进入phase I应该是很快的事了。
话说回来，有一点我个人比较确定，ARC-5单纯作为monotheray是不大可能真正解决问
题的，必然是需要和其他现有的antivirals或immune modulators联合使用或者交替使
用才能达到最佳的效果。对于非活跃性的慢性感染者尤其是经过了一定时间抗病毒治疗
的患者而言，有很大一部分其实只有大约30%左右至更少比例的肝细胞存在HBV感染，而
肝的再生能力是超强的，所以理论上这绝不是什么问题，当然，如果能实现非杀伤性的
cccDNA清除那是最理想的，最好能mimic你所说的成人急性感染后病毒清除的机制。只
要能打破耐受，后面一切皆有可能。
至于你所说的二期实验至今未发现HBsAb阳转，这个其实是意料之中的事情了，毕竟尚
未实现HBsAg clearence，那么指望seroconversion自然是不现实的了。目前需要等待

我就把去年的贴里找点出来重新贴一下吧。
GAS当然很重要，但是文章去年才发，对于他评院士应该不是最重要的贡献。
个人觉得他最重要的发现是发现ubiquitin signaling不光可以诱导蛋白降解，也可以
激活信号通路，而且是一个非常general的mechanism.最早他是在NFkB领域提出的。而
那个时候IkBa的ubiquitination导致IkB降解已经是非常经典的机制，所以当时他的理
论被认为是匪夷所思，被一帮老学霸如MK等打压。后来被多个实验室证实，而且在其他
信号通路比如DNA damage response中也是一种非常普遍的机制。

Based on my limited ENT rotation, she needs to be seen by ENT if not
done yet. They can do an audiograph to
see if it is sensoryneural. If it is she need to take steroids and maybe
antiviral agents to help preserve the hearing
function. It may resolve on its own too. This is the one diagnose I would
rule out before considering any CNS causes.
I hope ur dr. has already done that. If it is not done yet, worth a try.
Again this is just based on my limited ent rotation.

questions since it varies depending individual patient and the competency
level of the medical personnel. You may want to ask the specific facility
and physician who is or will be taking care of your Dad. I do believe he
needs lifelong antiviral treatment after the liver transplant. |

share 资料的方法还要想一想，我这次用的share 一个文件夹，
但是公共账号的被删了我自己账号下的也没有了。
其实学校混，还有找Intern是Asia 看Asia顺眼，
尤其是我们第一代移民。
学校里study group大部分是按种族分开的。
虽然大家都客客气气，
最后关键时刻还是能看出来不同的。

.... IV相容肯定要考的，rxprep里都有的。
最近一两年FDA批准上市的新药要学习，我有被考到。
Chemo,antiviral 我有被考到。但都是出现在考试非常后面，也许是前面对得比较多后
面就开始出变态题了。
Pharmacy law那本书要学习，虽然只有一两道题，但绝对不能错。

metoclopramide
以下是肾功能较严重不足就要注意避免的一些药物：
metformin
duloxetine
nitrofurantoin
NSAIDs
K-sparing diuretics
lithium
xarelto
bisphosphonate
CKD的病人如果有高血压或者糖尿病，应该使用ACEI/ARB，不过医生好像常常忽略~ 药
剂师在做chart review的时候不要忘记检查哦。同时还要记得monitor serum
creatinine and K+. |

unilateral HSV-1 infection. The HSV-1 infection associated autoimmune
process explains why the infection was unilateral, but the bell's palsy was
bilateral. So steriods could inhibit the immunoresponse but antiviral
treatment would do nothing about it. Works for prevention? maybe. |
