发帖数: 26 | 1 我在国内体检胸腔x-ray发现肺右上页肿瘤，手术摘除肺右上页。
肿瘤大小是9X9X3cm，已经侵入胸腔肌肉。PET/CT没有发现其它部位有肿瘤。
病理分析是小细胞肺癌。出院后回美国治疗。在美国的病理分析结果是，atypical
lung carcinoid，PET/CT显示胸腔上还有肿瘤残余，也可能是手术后的炎症（我背部还
有疼痛感，距手术2个月），但医生更倾向于肿瘤。Bronchoscopy, cervical
mediastionoscopy 结果没有发现向淋巴结的转移，MRI没有发现脑部肿瘤。治疗方案是
化疗4个疗程（cisplatin+etoposide），中间有放疗。医生担心这个肿瘤长得太快（一
年前x-ray没有发现），所以按照小细胞来治疗。
我在想是不是可以等一等，确定胸腔上的手术伤口愈合后再治疗。如果PET/CT的影像不
是肿瘤，
就不考虑放疗了。
不知我这个想法是不是有道理，请给位医生提供指导意见。 |

发帖数: 413 | 2 这位怎么象吃了火药一样？我不是医生，当然知道自己没法下诊断。之所以这样怀疑，
是因为有的医生实在太不靠谱。儿医8月6号看血检结果还说，没问题，病毒感染，血小
板比正常值高，白血球高，是正常反应。突然让我们14号去查血，说是因为便秘，有一
个星期学走路扭伤脚，有点点limping，根据这两个他觉得要做血检排除neuroblastoma
。我们一再强调，limping早就好了，便秘也好了，结果还是被建议查血查尿。
你觉得atypical Kawasaki disease 是我自己瞎想，那这个neuroblastoma是不是更无
稽？ 用今天urgent care的doctor的话来说，是shooting in the dark。医生就都靠谱
？病人及家属就不能 be educated? |

(一年前x-ray没有发现), this is the key point, it is more than a regular
atypical carcinoid. |

1. atypical 不能算诊断。
Answer:
The DSM-IV-TR defines Atypical Depression as a subtype of Major Depressive
Disorder with Atypical Features, characterized by:
a) Mood reactivity (i.e., mood brightens in response to actual or potential
positive events)
b) At least two of the following:
Significant weight gain or increase in appetite;
Hypersomnia (sleeping too much, as opposed to the insomnia present in
melancholic depression);
Leaden paralysis (i.e., heavy, leaden feelings in arms or legs);
