在的pd-1/pdl1, 病人有约50％ 的反应率，而且在多个肿瘤类型上有效。其他的免疫治
疗方法都发展很快，而且都逐渐出来效果了。dentrian 的cancer vaccine， 2010年批
的，虽然效果一般，但是是免疫治疗的一个里程碑，后续的很多改进了疫苗将会逐步出
来。
免疫治疗目前都是用在晚期病人身上，往前推，效果应该会更好，在副作用和安全性方
面，总体来说还有不少问题需要解决，虽然pd-1的副作用比ctla-4下降了很多。
特异性抗原是一大难点。不过杀死以后另外一批又浮现出来，却是免疫治疗的优势，免
疫系统的最大有点就是adaptive, 只要免疫反应足够强，免疫系统会围新出来的抗原激
活出新的免疫细胞，从而实现动态控制，这时免疫治疗最大的不同，从ctla-4的数据就
可以看出，长期存活率提高了15％左右，就是说，一旦免疫反应达到一定强度，实现了
对肿瘤的清除和控制，复发的几率将非常非常低。

种种所谓经典理论，都基本被重写过多次，这点自然也是尽人皆知，所以我在表述前加
了一个很明确的暗示，“通俗的说”，我相信你看的懂这里的意思。
换句话说，通过checkpoint blockade打破之前的免疫耐受以及相应的肿瘤微环境，在
innate/adaptive immunity的共同作用下期望可以有效限制肿瘤免疫逃逸，这是目前免
疫治疗的基本原理之一，临床效果为证，至于如何商榷，当然没有这么简化，那就不是
我说了算了。
纠缠肿瘤的免疫抑制究竟到底是因还是果，其实在我看来不重要或者没有意义，因为这
必然是因果相扣的，但是消除免疫抑制有助于防止肿瘤逃逸，这点没有必要再纠缠吧？
当然还是可以列出一大堆文献，但是免疫学不就是这么矛盾吗？
