的妇女中会发生。这个主要是因为St.Johns wort对肝脏消化酶有很强的作用， 尤其是
CYP。 很多药物的代谢都是经过CYP酶。 所以如果同时服用其他药物， 服用St.
Johns wort前一定要咨询医生或药剂师。
St.Johns wort在欧洲很流行， 很多医生会开这个药治疗青少年轻度至中度抑郁。主要
原因是青少年一般应该不会服用其他药物。

你能找到一小群遗传背景对药物敏感的人做中试什么的，搞不好就过了那道坎了，据我
所知这正是pharmacogenetics最近热点之一，药厂是相当热衷的。不过大家要搞清楚，
这个其实根本不是传统意义的pharmacogenetics，大家一般其实称为target therapy，

以我作为病人家属立场而言，对你这个基因测试的建议不屑一顾，原因很简单，吃波立
维就是因为已经对aspirin不耐了，波立维是目前唯一的替代选项，即使基因测试出它
没有效果又如何，又不是说测出它有副作用，我不会因此改变我爸的treatment plan。
何况基因测试只是测无穷多个CYP基因里的一种，万一有别的现在还不知道的基因可以
消化clopidogrel呢？如果是做药代测出没有消化还比较有说服力。基因测试太新，不
成熟！

题库：uw两遍，Rx一遍，Kaplan一遍。还走马观花的做了Pretest的很老的书（当年脑
袋一热就买了，本以为这辈子都用不上了，为了不枉自己花钱，咬牙做了，不过不推荐
，题偏）和Robbins pathology。
4.知识点：说实话7个block下来前面考过什么都不太记得了。就把自己记得的说说：

，很多职位并不需要phd。analytical目前是会biological mass spec的吃香。小分子
PKPD modeling目前还行，但现在这类职位都prefer会
大分子modeling的。如果会一些modeling又有吸收代谢之类assay的经验，应该比较好。 |

发帖数: 34 | 45 非常谢谢lilybank的回复。病人最近的BNP是1050， 对于房颤病人这个是否很高或者正
常。其他检查都没有问题。病人饮食也正常。amlodipine是病人必须用的降压药，因为
只有这药对病人血压有效。血压不是一直都低，是一天的某个时段。可能需要减少
amlodipine或 Irbesartan用量。停用statin的4天里，INR就从2.27降到1.29。可见华
法林与其它药物的相互作用是很明显的。
我把我最近查到的跟大家分享一下：
涉及的华法林的代谢CYP450同工酶包括CYP2C9，2C19，2C8，2C18，1A2和3A4。更有效
的华法林的S-对映异构体是由CYP2C9代谢，而R-对映异构体是由CYP1A2和3A4代谢。
amlodipine是3A4的抑制剂。瑞舒伐他汀（Crestor）经CYP2C9代谢。所以停用了瑞舒伐
他汀，对法华林代谢抑制减少很多，INR明显降低。
大多数他汀类药物通过细胞色素P450（CYP）代谢途径代谢;阿托伐他汀（
atorvastatin），辛伐他汀（simvastatin），洛伐他汀（美降脂，Altocor）通过
