Diagnosis of epilepsy is reached, that means no identifiable cause is found.
Based on the child's clinical manifestation and EEG, seems it's complex
partial seizure. Several DDXs at this age include infantile spasm (EEG
should be much more chaotic), absence (lasts only seconds instead of minutes
in this case, and EEG is neither 2.5Hz for atypical nor 3.0 Hz for typical
one). |

one of the ddx. either renal sono or CT A/P should be done, esp. if pt is
hemodynamically unstable. All tests can be done within a few hours. Image
test is not necessary for diagnosis or treatment for pyelo, but if pt is
critical or there is no improvement after treatment started 24-48 hr, or pt
has a lot of co-morbidities, imaging should be done immediately. My 2 cents
.

Step3与流水账
时间：准确的开始日子是2010-11-14，结束日子是2011-1-25日，历时72天。其中11-14
～1.1日以Mark & Jason
Family medicine为纲，外加UW Qbank MCQ，复习Step3 MCQ内容，间插小组讨论；1.2
日模考NBME，1.3日－1.22
日以UW CCS为纲，复习Step3 CCS内容。1.23日模考UW。1.24考48题×7个block MCQ，1
.25考36题×4个blcok＋9
个CCS case。复习时间是每天2个小时，周末4个小时，Thanks giving＋X'mas＋new
years大周末6-8个小时/天。

一起在颤抖。“Congratulation! You have matched” 真不敢相信，我居然match上了
。在第一时间给太太打电话，发觉自己的声音都是颤抖的。三天后的Where did I
match已经是无关紧要的了。能够match上对我，step1/2仅刚刚pass 80，来说已经是够
幸运的啦，去哪儿都是无所谓的。在一年多前我拿到step1的成绩时曾苦恼得来版面上
寻求建议，当时版面上的一个著名的粉牛粉牛的ID回了帖子说我几乎是没有希望了。
想想过去这些年我的烤板之路，怎么说呢？我是起个大早，赶了个晚集。一路走来，经
历了家庭、生活、和工作的种种变故，居然能够进入这一扇门，我应该感谢的是我太太
120%的支持，在一切一切的风风雨雨中不离不弃，咬牙坚持。这一句“Congratulation

FA closure 不太理想，最好勤加练习，与Partner互相总结；
partner找个非中国人挺好，利于互相学习；
去CS辅导班之前最好对CS初步有个底，才能吸收到更多东西；
CS 可以去网上熟悉PN界面，似乎还有地方提供mock exam |

There are different types of porphyria, and a blood test sometimes gives you
more info. I remember I ordered a bllood test then for the diagnosis.

case, Zofran usually does not cause EPS. It was a serotonin agent( which
very rare causes EPS). Reglan/compazine are D2 antagonist.
If you do not consider Seizure as a differential diagnosis, your
consultation is incomplete. D2 antagonist lowers seizure threshould and
ocular movement could be aura or non-convulsive event(not necessorily affect

在每天跑步的时候就叽里哇啦地背。这里顺便评价一下，Arthur教的protocol和
group question很实用，病人都能听懂，但information sharing部分稍微
简单了一点（我是borderline过的），如果你在最后感觉还有时间，语言也还行，
可以多解释一点，我没解释太多，怕画蛇添足。

the money.
There are three major parts in this workshop. One is general lecture
covering some general info and major Systemic Qs. Second is case review,
which is really helped a lot on understanding the cc, DDx and focused PE.
The third part is the SP encounter, in which you will get the feedback from
SP immediately after encounter.

过个2－3遍。比较快的是跟partner讨论，可以把minicase的右面两栏给盖住，每个
case，都写出5个ddx和workup，然后两人讨论。其实最快的方法是自己过了，用同样的
方法，过几遍，对考试很有帮助。

历，没有太多惊喜，而更多的是百感交集。潜水多年，把我的经验教训写下来，希望对
后面的考生有帮助。
我于去年4月中旬改题前约了PHILLY 考场， 一来离家近，二来其他考区合适时间点已
经订满。考前去了NJ 2天班，也跟小组练了近一个月。从平时工作和老美交流中感觉语
感和表达都不错，也不乏听到赞美之音。考前担心更多的是记不住PROTOCOL，漏太多问
题。考时有点小紧张，但总体觉得还成，一切照着诸多考经描述中进行。记得有个SP
甚至还在我离开前微笑着说"I APPRECIATE IT"。考后如火如荼地进行着OB。结果FAIL
，整个计划打乱，无法再约上MATCH 前出成绩的考试时间。可以想象当时的愤怒，郁闷
甚至想到申诉。前段时间看到一帖就和我的情况非常相近。我也FAIL在SEP上，SEP平均
点就落在BORDERLINE BAR左缘上。看到那么多擦边过的同学，我没有那么幸运。考试由
12个SP 评定，评估结果应该是可信的。一切静下来后开始好好总结问题出在哪里。

的DDx记住。
first aid CS的句子先都背下来吗？如果能背下来会有帮助。Kaplan 课上也会教你该
怎么跟病人交流。
另外你可以看看有没有KAplan CS 网上课程，一般报CS 5-day course 送CS 网上课程。
Warning: Kaplan CS 网上课程不怎么样，如果免费送的你的听听当预习。如果要另外
交钱就算了。如果网上老师教的和Live class 讲的有出入，以LIve class的为准。 |

. There were too many students and in the whole 2 days I only practiced 5
cases with 3 SPs. Hard to believe.
I agree his DDx and PN parts are good. However, if I have known there were
more than 50 students in the class, I would NOT go.
Some of my other friends told me that Dr. T had a very bad temper. If he is
in bad mood, he will treat students like trash. |
