大家知道belladonna吧， beautiful lady的意思， 因为这玩意可以把pupil dilated
， 让women 好看。 为什么？ 因为它含有atropine。 大家都知道扩瞳是
anticolinergic effect， 所以如果知道belladonna的beautiful lady的典故， 傻子
都可以从belladonna中萃取atropine。
青篙素是从青篙中萃取的， 怎么萃取的真的不是那么重要， 重要的是为什么选了青篙
。 青篙如果不是中药， 如果没有相关的药性， 屠老太太她再聪明，也不会把它放到
那200种草药中筛选。
科学当然需要运气， 但偶然性里蕴含了必然性。
opium含有morphine， 知道了morphine的结构， medicinal chemistry就可以把它变构
， 搞出成千上万的衍生物， 然后screen出更selective， powerful的镇痛药物。 如
果要论功给赏， 发现opium的止痛效应应该是首首功。
在青篙素的发现上， 我们应该给中医credit。 |

物，而植物是客观存在的。跟有没有中医没有关系。并不是说有了中医的理论了大家才
找到了青篙。
不要替中医冒功。

已。 我觉得这个例子应该是中国人的悲哀， 明明知道这么多中草药有药理作用， 至今
只有青蒿素是唯一一个被全世界接受的从中国出来的药
随便说几个洋人发明重要药物的例子：
远的 一百多年前，洋人发现柳树皮的粉能止痛， 后来就从柳树皮里分出Asprin这个千
古奇药
中期的 比如上世纪早期Fleming 发现青霉素， 救了无数人的命
最近的 比如上世纪中期 Wall从紫杉皮里发现paclitaxel， 没救人命， 但是延长了好多癌症病
人

myelopathy causing right arm weakness?

As you said the patient is hemodynamically stable. She did not need systemic
thrombolytic treatment for now. Even though, a bedside ECHO should be done.
She had new onset RBBB, and leaking troponin which showed that her right
heart was under a significant stress from her masssive/submassive PE. It is
a grey area, wether she should get systemic or cath directed thrombolytic
treatment if she have RV dilatation or hypokinetic movement. |

应该来说，自己的情况还算好的把。2年前体检出有早博，医生说正常就没放心上。小
时候有偶尔几次心脏不舒服，不懂没在意。这次是听诊，心电图，后来做的经食道检查
，主要结果是约10-12mm继发孔型房缺，右心室Moderately dilated，肺动脉收缩压约
25 mmHg，左右--分流significant，室上性早博，不是内行，可能翻译的不太准确
既然增大的右心室能恢复到normal size，其他也都能恢复，我就放心了。原来是担心
等医生休假回来如果积累了很多病患还要排队等待，刚确诊时也蒙了有点着急，怕长时
间等待会对病程有影响，造成质的改变，既然只是量上的，而且可逆，就无所谓了
后来从就诊的医生那知道他联系的是个比较牛的专家，就决定继续等了。自己再去换个
医生，也不一定能得到更好的结果，这都是难以预测的。已经决定做手术了，如果医生
认为适合做介入的话，查了资料觉得不能任由其发展了呵呵 |

clomid，clomid会造成我排卵提前，但是那specialist不相信这一点，因为之前我只是
用ovulation test kit 去测排卵。他认为我可能pickup了clomid.结果就是周期第11天
，发现我有3个大卵泡，但是lining只有4mm。大夫还说你应该过3，4天会排卵，让我预
备好trigger shot。结果呢，第二天就把我揪回诊所，因为我的验血证明要排卵了。所
以毫无疑问的失败了，连那trigger shot 也没用上。后来捐给了诊所。我当时都不知
