美国已经有 medicare 给老人, medicaid 给穷人,SCHIP 给不符合medicaid要求的孩
子,还有 EMTALA 要求医院急诊必须要给病人看病,不管你有没有保险。 |

院有义务救治。你可以仔细读读这个规定。还有美国的多数医院是愿意和病人谈判的,
像你这个情况,医院肯定能减免,而且还是大幅减免。 |

Rights.
The focus of this posting is emergency care, as well as itemized billing.

with medical condition being ignored because of ethnicity, rich/poor, origin
of country, etc. check EMTALA.
There are many patients in ER that have no insurance, Americans, foreigners,
Black or whites. Doctors work for the hospital got paid regardless whether
the patient has insurance or not. Your mom who suffered from a stroke may
not need any IV fluid, why do you judge fair treatment based on this?

与国内的同事朋友交流,他们对美国的急诊制度都会很关注。其中的一个主要原因是,
两者的看病付费制度截然不同。国内病人看急诊是要先付费,没有钱是看不了病的。我
也曾经问过朋友,那么如果病人真的是没有钱怎么办?朋友答曰,医院规定总值班签字
。一旦这个字签下去,如果病人赖账走人,最后是那个总值班要扣钱的。因此我们经常
看到病人入了急诊室,家属急着去交钱,钱不够,用不上药,医疗纷争就上来了。
在美国,急诊和住院都是先服务后收费。1986年美国通过一项联邦法律,EMTALA即
Emergency Medical Treatment and Active Labor Act。它要求医院不论病人的移民身
份,不论病人是否有钱,都要为病人提供所需的急诊医疗服务。这项法律是所有住院医
生及治疗医生上岗培训的必修课。那么,病人出院后的收费是如何处理的。医院有特定
的财政部门会负责向病人收取急诊或住院费用。病人出院后会在一个月内陆续收到医院

1. Participated Hospitals,医院可以选择参加或不参加EMATALA。比如Veterans
Hospital 系统的就不在EMATALA范围内。 但是美国政府有个巨大的胡萝卜,Medicare
和Medicaid。一般认为这部分开支占美国医疗总消费的40%以上。 不参加EMATALA就拿
不到Medicare和Medicaid。普通
2. Non-covered medical conditions。医院根据一定的procedure决定你是不是
Emergency。
3. Stablization. 病人stable后,医院可以Discharge他了。
说白了就是美国, 医院都有各种办法cover和Absorb这部分cost。而在国内, 医院做
