it is true that nowadays they don't require many more procedures as they
used to, which is sad, depends on what you plan to do in the future, if you
just want to be a clinician and see patients in clinic everyday and it
probably will be ok not to know how to do procedures,
if you plan to be a hospitalist, then you should at least know how to
intubate and how to place central lines,
if you subspecialize, i think endocrine doesn't have any procedures, but

6月2号考的，今天出成绩。
去年7月考完step1,然后生孩子。跟Damin,xiaofeng学了一阵儿，受益匪浅，非常感谢.
但是因为自己带孩子，不太可能有固定时间跟study group,就自己学习了。从去年10月
左右开始正式看，每天3-4小时。一些知识点模糊了，还得翻看step1 FA.
看了全部 Kaplan vedio,很受益。一些知识点老师讲的肯定透彻，根本不会忘。并在
notes上做了笔记。vedio里推荐endocrine, Cardio,neuro, emergency, 还有OB/GYn。
儿科vedio不推荐，基本上是念书。
建议看一章k-notes,再看一章MTB CK.因为MTB归纳总结的好，可以帮助理清思路。（这
是教训，我是最后几个月用的MTB）

Thus TSH must be done to r/o [rule out] the endocrine cause.
One does not need ALL symptoms/signs to suspect a clinical diagnosis. This
pt's picture: decreased DTR, relatively slow HR (<60/min is bradycardia) is
consistent w/ hypothyroidism.
Isn't it common for someone who has poor sleep to lose weight?
I am not saying hypothyroidism IS the Dx.
But it IS a Dx that MUST be entertained and r/o, before starting anti-
depression Tx.

Specimen of right breast mass 4X2X3CM was sent for inspection. A grey nodule
with size of 2.5X2CM was found in the section. Endoscope finding: The tumor
cells were in nest shape and glandular tube. The cells arranged solidly and
grew invasively. Nuclear fission was easy to see, and nuclear atypia was
obvious. No cancer metastasis was found in the 3 pieces of lymph nodes in

住院医讲的全是近期医疗事故，我想这是她连本院的endocrine fellowship都申请不上

性的，谁都不喜欢天天负能量的人。

下个月打算用一个月的时间弄一下kaplan题库，回来再用3周做一遍uw。
现在属于，对知识理解纵深方向努力阶段，还有就是查漏补缺。
我发现自己以前对知识理解的不够precise，还有就是对知识的把握不够宏观。
节奏：
Endocrine，GI，Hematology，Muscle，Renal，Reproductive，Respiratory，3-4天一
章。Cadio，Neuro，Micro配合其它的学习一周一章。
中间也在穿插着看First aid step2 ck, 8thEdition, MTB2 and Yale-G's Refined
Clinical Knowledge for the USMLE Step 2&3。
各个章节都在记忆以及搞难点，就是differential diagnosis不清楚的点。
药理打算再仔细把各章弄一边，不过速度应该都比较快，主要是带着问题回答问题。我
可以focus的时候做题速度比较快。
期间打算把pathoma 2x速度再配合题库听一遍。

适合人群：
全部单科复习完一遍 或者FA过完一遍，开始做题阶段。目标是年底考试的同学。
具体目标：
完成FA 2016 Organ System，完成 kaplan bank 。
要求和具体学习方式：
必须能够保证跟进小组进度。要求每周按进度复习一个FA Organ System。 然后做相应
的kaplan qbank题。小组学习时间每周两到三次结合FA。具体小组学习时间： 每周三
四五 at pacific time 6:30 到 9:00pm. 或者每周六 pacific time 8:30am 到 12:

I remember that type 1 is the insulin dependent which means pt don't secret
insulin at all. Since the question was talking about not enough insulin
secretion. I think it should be Type 2 and it won't cause ketone acidosis.
How you guy/gals think about the ans to 48? |

dependent DM), it'll go to ketoacidosis when B.S. level is still around 400.
And type 2 (NIDDM) can secrete enough insulin to keep pt our of keto state.
choice 1 was out of the discussion because it is nothing involved with
hyperglycemia condition. :)

48. Diabetic ketoacidosis is maily caused by type 1 DM, but can also be
caused by type II. Thus, lack of insulin or not enough insulin. As for HHNS,
only type II causes it, not type I. Diabetes insipidus is caused by lack of
ADH, has nothing to do with insulin. |

insulin的不敏感（主要是肌肉和脂肪组织的insulin resistent）, 而不是insulin的
多少。题目问的是由于insulin的量产生的影响，应该就是说type 1 diabetes。 另外
，type 1 diabetes 是由于胰腺β细胞坏死退化，insulin产生急剧减少，但是并不一
定是0。 |

多10道medication，2道calculation，一些endocrine，infectious control的题。
有个问题，什么样的题算难题？除了select all，题目里把priority, immediately,
first, most这样的字全部加深highlight，这样的题是难题吗？
我的pattern是前10几题都是单选，topic都知道，没有不认识的病，但很多都不确定，
其中有2道药题。10几题后出现一道多选，然后就是普通的题加priority交替。我发现
我确定对的题后面基本上是priority和select all。最后几题全是priority，自我感觉
应该对，但也有不确定的。最后一题after peritoneal dialysis, which observation
need first follow-up: n and v; fatigue and xxx; anorexia and xxx; straw-
colored dialysate drainage |

感觉：考下来感觉难度一般。个别问题觉得没有正确答案，个别问题完全不知道怎么答
。大部分问题还是有底的。
考的范围很广，我的考卷涉及到的题目有：
math 10%(网上有人说会占30%,可能他们错太多，所以naplex系统给他们题目直到做对
为止吧), prescription evaluation, compounding, kinetics, biostatics, heart
failure, arrhythmia, hypertension, anticoagulation, lipids, diabetes,
endocrine, women's health, ADHD, asthma/COPD, ESRD, seizure, oncology
