齿算长得快的，只剩一个baby teeth， 最近看了两个orthodontist，两个人都主张先
用expander将upper jaw 撑一下，但后续treatments不同：
一个主张expander的同时加6个上面牙齿的brace，疗程大约一年，然后等3年左右再用
full brace矫正。他说等3年再用full brace的原因因为孩子的牙齿还在长，等3年牙齿
固定了再用final full brace。 他还说如果第一个expander效果不好还要用second
expander，另外矫正不好的话还会要动jaw的手术。第一个疗程（expander+6个牙齿
brace）要3800美金，如果需要第二个expander另加1000美金。 三年后full brace要另
外4000美金。

这里说一下ACA主要条例和影响。 其他的对Medicare的影响， Accountable Care
Organization等比较不relevant和影 响比较小就不说了：
1. ACA原来规定每个州都必须expand Medicaid， 但是后来给supreme court否决， 说
联邦政府没权这样去要求州政 府。 州政府可以决定是否要expand Medicaid。 然后大
概有一半的州（比如加州）就根据ACA来expand Medicaid。 另外 一半（比如德州）就
没有改变Medicaid。

我做题的顺序以及分数，供参考。
1. NBME step 1 form 1: offline 正确率 70% （各科书看完一遍后）。 但是做完看
题完全没有clue, 也不知道所谓的正确答案正确与否。跟sahara 讨论了一下题目，有
些得到了解答，但是总的觉得自己还差很远。每一道题都要花很长时间寻求答案，写了
很多notes,后来觉得还是先做题再回来看。
教训：可以做NBME forms 看看自己的水平以及弱项，但这时不适合花太多时间研究
NBME 题目。
2. Kaplan Qbank (2-month): 看完一遍Kaplan notes 后, 正确率 59%。
3. NBME form 6: online with expanded feedback 440/209 （76% 正确）(考前8.5
个月）。 之所以做form 6, 是因为form 6 有expanded form, 可以知道那些题做错了。这
样不用把所有题目都研究一遍。 因为想着以后找NBME 小组的时候再好好研究题目，所
