心里好难过。。。19个多月的小毛蛋儿，一直很健康。昨天中午玩儿回来就莫名其妙的

了氧气，清醒过来，然后去了ER。折腾到12点，回家观察。试图抽血，可是孩子太累了
，一直挣扎，扎了两针都没扎到静脉。
今天还在烧，最高到了102.2，医生让在家观察，给药，随时联系。
目前怀疑是7天前打得疫苗MMR（还有DTap，水痘），可是当时打针后的那个晚上就烧过
了，转天也好了。或者是细菌感染，但是没法抽血，不能确认。
不知道为什么一定要抽静脉血。像孩子现在这么不舒服，见了针管、病房就害怕的，该

information I re-wrote.
1. Febrile seizure usually occurs in children younger than 6 year old.
2. If the patient has status epilepticus (持续抽搐, continuous seizures or
intermittent seizures without neurological recovery), it could be a special
subtype of febrile seizure, called febrile status epilepticus. Most ED
physicians are not aware of this (never taught in medical school, nor tested

the normal population (kids). ED management depends on whether it is a
benign, simple febrile seizure or a complicated one. The kid's age plays an
important role too. If somebody is < 6 month old, I won't call it a febrile
seizure to start with. By and large, it is a diagnose of exclusion. There
should be no developmental concerns, no CNS infection, no metabolic issues,

understanding of febrile seizure in kids.
1) THIS IS NOT YOUR FAULT. Some kids tend to get febrile seizure more often
than others. This is genetically determined, not a lack of care. Although
most kids with febrile seizure resolves by themselves, a small portion of

seizure activity secondary to rapid increase in body temperature. Most kids
with febrile seizure do not develop seizure disorder as they grow up.
However, seizure disorder occurs at a higher incidence in those who had

It is hard to tell whether you daughter is developing seizure disorder based

Avoid high temp!!
Said 3% of all baby.
But most will recover after 5-yr old.
Possible long term damage if last > 5min.
More severe if it is a focal-type, i.e. uneven seizure from left/right sides
of the body, eye tile etc.
May track family history...
Said if happened again won't consider as febrile seizure.

我大儿两岁半和三岁半都有一次 febrile seizure， 之后就没有了。医生说，5岁之后
高热惊厥应该比较少见。我小儿三岁时有一次高热惊厥，5岁还差三个月时，发烧100度
以上整24小时都没有抽。 |

这个看着很吓人，但是及时护理的话很少会有长期不良反应。我小时候就是经常生病，
发过好多次febrile seizure，基本上后来我一发烧我爸我妈就立刻送我去医院。我现
在还有小时候做心电图还是脑电图的记忆。我不记得最后一次febrile seizure是什么
时候了，但是记得在五六岁的时候我爸又送我去医院，医生说这么大就不用担心了，不
过那次还是留我住院了，也的确没发seizure。那是我最后一次住院，从那以后基本上
没怎么生过病，自我感觉是脑子没烧坏，:P |

UW 41 offline cases: 我当时和partner每个case仔细做一遍，虽然不像online有反馈
，但对练临床思维，补充protocol非常有帮助。最后考试与41 case有异曲同工之处，
遇到复杂case也不慌神。

看看我家宝宝cpt2的两个结果， 一个1.5%酶活性，一个15%酶活性，跟下面的温度相关
吻合得多好。
我家宝宝的下降更多，大概是因为他的变异更多。这个特征的变异很多年了，但是
cchmc和Baylor都没有查出来。你说说他们不知道？！

, 不吃退烧药，病程要短很多（3天左右）。一般来说，如果小孩不是有febrile
seizure的体质，high grade fever (higher than 39C)也不用尺退烧药， 高烧的话，
好得更快，一般一到两天就好了。
吃了退烧药，病程就较长，而且会比较难控制，七天到十天好都算正常。两周以内不好

seizure. Most of them will resolve when child gets older. The most important
thing is to control fever - give tylenol/motrin aroud the clock whever she
develops a cold. However, if it occurs very frequent, some pediatric
neurologist will give anti-seizure meds for a few years then stop it. You
should consult a pediatric neurologist. |

virus或者感染。医生说白细胞数量还是很高，也许是有某些感染他们还没发现。不过
现在体征已经很平稳了，也有一点意识了，就是还不太能控制自己的身体。没有完全清
醒。不知道还需要再等多久才能恢复。

其实不一定高烧，如果发烧体温短时间迅速升高就容易因为惊厥，很多小孩都经历这个
阶段，大了会outgrow。
这段时间发烧了要按时给药，尤其是晚上，不能bundle up，发烧了把衣服脱掉。。。
泰诺和iburprofen 3-4个小时交换喂。
很可能是MMR疫苗，酱紫的话应该2-3天就退烧了。 |

了我们很多。
怀疑就是疫苗反应，加上之前感冒发烧刚刚好，发烧前几天都玩儿的比较疯，所以体力
比较差。

觉得有必要作为经验教训的是：
孩子发热期间，还是不要让孩子大喜大悲，一个是容易吐，一个是容易体温骤然飙升（
这次的高烧晕厥可能就是因为太高兴了，体温一下子上去了）。因为这个高烧晕厥（
Febrile seizure）不一定是因为绝对温度，还有一个相对温度的变化。
孩子高烧，真是要及时想到物理降温，温水擦下，比较温和也有点用，多喝凉白开。特
别是刚刚开始烧，不打算给药的时候，也要注意观察体温变化。
孩子要晕的话，及时掐人中。
最不过，真的晕了，可以侧躺在地上（我们租房刚好地毯，够硬又不凉），确保孩子呼
吸顺畅。
这次毛蛋儿发烧有件事情特别奇怪，事后知道其实很危险。那天孩子睡午觉，可是20分
钟就醒来了，然后静静的躺了10分钟（一般他可是呆不住的，马上下床或者坐在那里玩

次烧得太高(超过104F)还来了一次febrile seizure,可把我们吓坏了.想问问大家有什
么办法能帮她增强免疫力.
宝宝每天运动还挺多的,不停地跑来跑去,所以我们觉得她容易生病不是因为缺乏运动.
想来想去可能有以下两个原因：
一是她太瘦,体重只有5%左右, 曾经有过一次大病之后体重降到3%以下.身高倒是一直在
80%到90%之间.现在回家后我们已经尽量喂饭了,但就是吃不多.白天在daycare吃的更少
。给她吃pedia sure也不太爱吃，真是伤透脑筋。
二是可能去daycare之后睡觉少了.以前在家是晚上睡10个小时,白天午睡2个小时。去
daycare通常午睡只睡半个多小时，每天至少少了一个小时。我们也想让她早点睡觉，
原来是晚上10点睡到早上8点，现在尽量让她9点就躺床上，可是人家总是可以在小床上
玩一个多小时，所以还是到10点以后才睡着。我们双职工，下班都比较晚，很难做到8
点以前让她睡觉。不知道怎样才能缩短她的入睡时间。

发帖数: 156 | 42 是不是febrile seizure? 重要的是降烧，Tylenol(acetaminophen) 和 Ibuprofen每三
小时轮流吃，再加上物理降温，用小酒精棉片(alcohol cotton swab)轻轻地擦脖子上
的主动脉，这样有利降头部的温度，同时用凉毛巾敷额头，用潮的毛巾擦手心和脚底心
。温度要尽量降到98F左右，但是acetaminophen 不能吃太多，注意一下Tylenol使用说
明，要轮流和Ibuprofen吃。酒精棉擦的面积不能太大，就只用在脖子主动脉上，也可
用在大腿根，但不要太多。
同时注意补充含电解质的液体摄入，可以用Pedialyte.
有条件的话打电话问一下中国的儿医或急诊大夫，他们比较有经验。以上是我在国内当
过急诊主任的岳父以前教我们的。

hypersensitive hypothalamus下丘脑that's responsible for body temperature
control.
但是以后要注意防止高烧，要提前给降烧药（注意不是抗生素），不要等到39－40C才
给药，一般一旦超过38C就要给药，然后控制温度不超过38C。 |

久的癫痫症，而是怕choking。基本上没有其他高危风险的febrile seizure发展成癫痫
症的比例是非常非常低的，其他的高危因素包括小孩本来有一定的delay，或者其他神
经系统发育问题,或者seisure的持续时间特别长,或者在一次生病期间重复发作的。如
果出现高危情况，应该和specialist谈，像楼主这样只是发了一次seizure，然后
seizure持续时间就1-2分钟的不用太担心。 |

时已经是半夜了，考虑到去emergency room弄不好又要等5、6个小时，所以给娃洗澡降
温后就直接让娃睡觉了。
周日早上娃烧退了，但我还是带娃去了urgent care，医生简单看了眼耳鼻喉后说是
virus flu, 当时给开了抗生素就打发回家了！吃了药过后娃的情况没有好转，反而烧
的厉害了，这让我很担心，于是预约了周一早上11点的儿医。这已经是我能预约到的最
早的时间了！晚上又给娃洗了好几遍澡，烧退了。体温正常到98F。
周一早上依旧不发烧，于是我们愉快地看了医生。医生看后表示这是手口足病，吃点退
烧药就好了，还表示抗生素一点用都没有！医生说她一点也不担心！还告诉我烧个两天
有时不用吃药自己都会好的，让我不要太紧张！ 于是我们真的就不紧张了！
在回家的路上我决定去家旁边的肉店买点肉给娃做点好吃的，正在肉店买肉的时候！我
突然发现娃不对劲了！她双手紧握，眼睛上翻，失去了意识而且身体不停地抽搐！当时
的症状真的很吓人！我第一反应她是食物卡住了，她当时正在吃零食，之前在常识里学
