按带状疱疹的治疗原则进行处理。
1. 疼痛先试用GABA类似物(gabapentin, mszzeta提到的pregabalin是两个常用的药物)
和carbamazepine。 局部麻醉软膏(比如利多卡因)也可使用。
2. 医生要决定需不需要用抗病毒药物。
还有一些其它的治疗方法用于疼痛严重又药物治疗无效的情况，包括高度选择性神经切

痛。现在医生开了gabapentin止疼，但是效果不大。这种神经痛非常痛苦，我查了美国
的Lidoderm, lidocaine 5%贴片可能管用，刚才查了版上的信息知道按常理国内的处方

缺乏会有手指，脚趾麻木，神经痛
美国这里singles就用抗病毒的治，gabapentin治neuropathy,外加一些激素在愈合期用
以减少疼痛。
在这里如果发现shingles， 一般都要做HIV test, 因为在免疫力非常低的时候才会发
病。不过国内就多见了。 |

中午回去，没有上厕所。昨晚，今早和今天中午，总共看到三次了，还有两次没看到的
，就是她进厕所了，比划了一下，没尿，然后自己埋了个小堆就出来了。
中午不肯吃东西，好不容易骗了几口干粮。灌了2ml的水。罐头不吃，最爱吃的酸奶舔
了几口。
昨天晚上开始吃打虫药了，Fenbendazole,每天一次。
另外，今早开始换止痛药，现在用的是Gabapentin，每次0.1ml
这会影响胃口和上厕所情况吗？
中午开始还感到有点烧，不知道是体温真的高，还是刚睡醒。不过今天感觉是没有昨天
活泼了。
怎么换了止痛药反而糟糕了呢？就算抗生素不管用，止痛药说是更strong了呀
老天保佑啊！！！！！

发帖数: 55 | 32 其实我不知道如何回答你的问题，因为如果我给了你建议，你如何去实施？止痛药物都
是控制药物，你个人很难得到，而且家属过多介入到治疗中，会令医生反感。而且我不
确定美国的一些药物国内是否应用。以下仅供参考
1） 烧伤疼痛治疗很难。换辅料时，针剂可以fentanyl + versed, 有时候应用
ketamine会有不错的效果（需要医生护士有经验）。其它时间，短效针剂如Dilaudid或
morphine. 长效口服止痛药如oxycontin 或 MS Contin; 如果没有明显心脏问题，可以
结合methadone。 短效口服药物oxycodone或Hydrocodone等。后期可以加用gabapentin.
2）烧伤辅料千奇百怪，很难说哪个更有优势，要看病人愈合情况调整。磺胺嘧啶银很
常用。后期为避免Pseudomonas或其它感染，用银辅料往往有效。因为液化，希望能保
证创面干燥。 一个月了，可以考虑异体皮移植了，最终还是要自体皮移植。当然因为
费用的问题，我不知道国内如何操作。

The TCA recommendation was not based on the Beer's list.
Antidepressants have been used for insomnia for long time, and TCAs have
been most rigorously studied compared to the other antidepressants. There
are a lot of review articles available for search through PubMed if you are
interested. TCAs are definitely appropriate options using at low dose and

以下是肾功能较严重不足就要注意避免的一些药物：
metformin
duloxetine
nitrofurantoin
NSAIDs
K-sparing diuretics
lithium
xarelto
bisphosphonate
CKD的病人如果有高血压或者糖尿病，应该使用ACEI/ARB，不过医生好像常常忽略~ 药
剂师在做chart review的时候不要忘记检查哦。同时还要记得monitor serum
creatinine and K+. |

will help if it's post-herpatic neuralgia.
We usually should always first try with Tyleno and NSAIDs, then Percocet
kind of medication. Like Nabla said, we need to be cautious to use
Amitriptyline, especially with old patients. If any other medication I
mentions previously does not work, we may think of Amitriptyline. Most of
Dermatologist don't use Amitriptyline for zoster. |

usually do not work well for PHN, which is one of the toughest pain
conditions to treat. Assuming this patient has tried multiple anti-
neuropathic medications already,

following dermatone distribution). Multiple studies showed promising results
of locally injected botox for PHN condition

pain所以对opioids反应比较差，治疗是multimodal approach.视肾功能情况，内科使
用gabapentin or pregabalin，介入治疗胸段使用肋间神经阻滞，腰段可以考虑
epidural
这里面提到的药物在美国无本地医生处方不可能买到，在中国都有卖。肋间神经阻滞需
要临床经验，严重副作用是气胸，血管内注射，accidental spinal injection,
allergic reactions, etc.
你百度一下附近的"疼痛科+肋间神经阻滞"。这个治疗一定要在有疼痛科的医院进行而
且要做好长期治疗准备。这个阻滞看病人反应，可能需要10几次甚至更多。
总之，你人在美国所做极为有限，买点多种维生素寄回去，也就这样了。 |
