发帖数: 2402 | 2 病人刚确诊是gastric adenocarcinoma. 普通全身CT正常，但是胃溃疡活检是gastric
signet ring cell cancer。PET/CT结果还没出来。
医生的方案是先化疗缩小区域，然后手术切除，最后再化疗。他说这种癌容易扩散，所
以先化疗。
请教是，这个方案合理吗？ 先期化疗三个月会影响预后吗？ 因为据说这个胃印戒细胞
癌对化疗不敏感呀。

前阵那个女性网友发贴，说她老公平时烟酒不沾，也坚持体育锻炼，结果突然检查出胃
癌晚期，并且癌细胞已扩散全身，之前一点征兆没有，我觉得比较蹊跷。
我不是学医的，但还是大约知道大部份癌症还是有征兆的。就胃癌而言，比较为世人所
知的征兆大约是：
1）食欲下降
2）时常胃痛或者胃胀
3）体重莫名其妙大幅下降
4）大便颜色呈黑色
一般医疗保险都会提供每年一次免费体检，家庭医生会做一些常规检查，晚期胃癌怎么
会逃得过医生的法眼？
如果胃癌从开始发病到晚期全程一点征兆没有，那就太可怕了。

回顾从step1自己闭门造车考了只带一个9的分数， step2开始参加小组学习，做了所有
能做的题只为拿个99，虽胸有成竹且考试感觉很好，最终但又得到一个9的分数。虽然
这次成绩不高，依然很高兴，毕竟不用发愁如果没过的话住院医开始后没有时间复习该
咋办。过去的3年中得到太多人的帮助 (此处略去人名20个)，绝大多数甚至从未谋面。
今天发贴也是为了回报我们这个社区吧。
自我介绍：毕业>15年，220+/220+/1st/210+；
复习时间：6月，头4个月每天约2-3小时，周末由于陪孩子们玩（前两年欠的太多，你
懂得）及做点家务（再不做但心被老婆炒掉），反而复习不了多少；最后2个月每天4小
时，鼓励自己考了这个2年内再不用受这个罪。

我爸爸有心脏病，不能进行化疗。
当时我们对此病一无所知，手术后才知道应该先检查幽门螺旋杆菌，抗生素根治后再手
术。现在医生说没有办法再检查幽门螺旋杆菌了，应该手术都切掉了。但是我们还是担
心有细菌残留，细菌会不会在手术中进入血液？我们想放疗之后进行抗生素治疗，应该
用普通疗法还是根治疗法？
还有就是手术后半个月多，我爸爸甲状腺出现柔软，无痛的肿块，CT检查为有两个有血
液供给的淋巴结。因为现在在专门的放疗科，人家也不管甲状腺的问题。
请问会不会是MALT Lymphoma转移到Thyroid? 我看网上说这种情况很少。那么会不会是
原发Thyroid Lymphoma? 可是是手术后才出现的。那会不会是gastric MALT lymphoma
已转化为large B cell lymphoma? 照理说手术切除的胃，医院应该好好化验的，结果
他们只做了免疫组化，根本没有检验是否有大B细胞，也没有明确说我爸爸的stage.
我们该怎么办啊？请班上的同胞们帮帮忙。

患者44岁，男，6年前因为肝癌做了肝移植 （符合米兰标准），之后无灾无病好几年。
去年2016年初发现轻微贫血，血色素12，因为当时在服小剂量阿司匹林，于是停服，看
看有没好转。托托拉拉到了年底，还是贫血，血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤，内镜医生直觉是GIST。随后CT，但CT却没有异常。4月初第二次胃镜
，GIST依然还在，取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

better to see a doctor. BTW, the antibiotics you mentioned are prescript

是造Capsaicin的原料。好处还不少，有抗炎止痛， 促进心血管循环，促进消化。我也
是到了美国才发现，从国内来的几乎没有不吃辣的（除了我），以前我室友， 湖南来
的， 吃饭就是西兰花加上老干妈。好处很多，当然如果有消化道溃疡的要当心，而且
也会可能对其他药物有干扰，比如抗高血压药，因为会产生catecholamine儿茶酚胺.有
兴趣的可以看看。

我爸爸有心脏病，不能进行化疗。
当时我们对此病一无所知，手术后才知道应该先检查幽门螺旋杆菌，抗生素根治后再手
术。现在医生说没有办法再检查幽门螺旋杆菌了，应该手术都切掉了。但是我们还是担
心有细菌残留，细菌会不会在手术中进入血液？我们想放疗之后进行抗生素治疗，应该
用普通疗法还是根治疗法？
还有就是手术后半个月多，我爸爸甲状腺出现柔软，无痛的肿块，CT检查为有两个有血
液供给的淋巴结。因为现在在专门的放疗科，人家也不管甲状腺的问题。
请问会不会是MALT Lymphoma转移到Thyroid? 我看网上说这种情况很少。那么会不会是
原发Thyroid Lymphoma? 可是是手术后才出现的。那会不会是gastric MALT lymphoma
已转化为large B cell lymphoma? 照理说手术切除的胃，医院应该好好化验的，结果
他们只做了免疫组化，根本没有检验是否有大B细胞，也没有明确说我爸爸的stage.
