1. Subcutaneous unfractionated heparin is used for prevention of DVT in
immobile patients or in hospitalized patients unable to ambulate. However,
after orthopedic surgery, especially after joint procedures, its efficacy is
very poor, given the increased venous stasis ,you should use warfarin.
2. Both chronic laxative use and chronic diuretic use can produce
hypokalemia. Severe hypokalemia, with plasma potassium <3 mEq/L, can

that's the case, SOB and pleurtic chest pain, tachycardia might be acute
onset of PE. if pt has nephrotic syndrome, these pts are very high risk for
PE/thrombosis.
watch his o2 sats, check EKG, if hypoxia cannot be corrected with high flow
o2 and pt persistent tachy, might need v/Q scan, CT with contrast is
probably not feasible now since he is probably having renal failure since
you guys sent him for renal biopsy.

probability but ct is negative.

没有人想到PE吗？半夜里把attending叫起来后，她第一个反应是病人又来了一次PE。
病人在做CT前就被上了heparin drip，转到了ICU。PT INR回来后是1.2. SaO2很好，没
有chest pain, SOB.
至于fluid，a-fib八成是和这个有关，病人的确如风mm分析的以前的echo有右心扩大，
LV的EF是正常的，我和senior谈了他还是认为要bolus病人。肺没有crackle，CXR没有
congestion. 脚也不肿，不太像是fluid overload. 现在病人还在run着fluid..... 我
不知道well compensated的右心衰是不是需要fluid restriction.
BMP回来后钾只有2.1，低钾会导致afib吗，不太确定。
BUN 55,Creatinine 1.2, 看上去像dehydration. 这个肾功能你们会让病人去做
contrast CT吗？

heparin is a no brainer at those situations, esp if pt has DVT/PE in the
past, and her INR is subtherapeutic.
also very important, she should be on lovenox lifelong as long as her cancer
is not cured, instead of coumadin.
but I am not that sure her rapid onset of Afib in this case is caused by
another epside of PE because she did not present with related symptoms(breas
swelling is her chief complain) and her afib only happened |

发帖数: 611 | 20 最后更新一下最新进展，attending还是决定hydrate病人后送去CT，CT结果是。。。。
。。。。没有PE. 病人on heparin同时加了coumadin，明天我去问问她为什么不用
lovenox.
cardiologist给用了amiodarine drip, 病人不久就恢复了sinus rhythm. 病人血压还
是出奇的低，对fluid没有反应，一度降到70，很吓人，但病人却没什么不舒服的。他
们还是给上了dopamine.
afib的病人自然是查了TSH了，呵呵，是正常的，troponin也正常。
明天又要on call了，看看会不会收到有趣的病人再来和大伙讨论讨论。 |

楚楚的，intern没有上级医生的指示哪敢擅自停heparin。
给PE病人tPA，挺新奇的，都没见到过。还有做angiogram，都确诊了还做啥，就算没确
诊，书上说是可以做，但好像也从来没见谁真做angiogram的。 |

１“How much bleeding risk will the bolus add to the heparin drip? ”我后来
去查了查文献，竟然没有查到，就想：What’s the possibility that such a
clincal trial exists？越想越觉得主治医都未必知道答案. 可当时为什么不反问主治
医一句:“What do you think?”结论是：因为我这一段时间被三年级印度MM训练的，
都不会反问了(详见第二段)。以后要改回来！
２印度MM这么当众揭人疮疤，难道那连柳下惠故事都有的印度古老文明里，就没有类似
“打人不打脸、骂人不揭短”的话么？
我曾经仔细想过，如果我是resident的话，应该怎么做？我想我应该：
（1）主治医查房时候能不提尽量不提，因为纵使患者真的出血了，你能分清是肝素持
续静点还是bolus 引起的吗？处理上又有什么区别呢？
（2）如果打算查房时候提出来讨论学习的话，我会先和 intern 达成一些共识，
（3）当众向主治医提出的时候，我会说“We gave a bolus”而不是“Th |

。有的vascular surgeon把手术后的病人上heparin,coumadin 抗凝，我不知道为什么

发帖数: 108 | 32 我做笔记还是原始的那一套。 准备一个索引本子，每隔几页编好号：A, B, C, ---。
另外的是笔记本，写好页码。做了题，读了题解后，觉得是自己不够清楚的，或觉得是
一个归纳的很好的知识点，就在笔记本上记下来，短的一两行，多的大半页。根据所记
的要点的关键词写写到索引本上，如 heparin induced thrombocytopenia，并其他可
能有用的关键词，如 Lepirudin, Argatroban 也可写人相应的栏内；其后写好页码。
在同一个字母栏内，就只能大概排序了。自然不可能像教科书那样好，但查找、串联起
来还是方便很多。半年、一年后要再用，查起来会更显帮助。当然时间是要多花一些。
现在的年轻人（我是老人了）都用电脑做笔记，应该有更快捷的索引方法，有待行家介
绍、分享。 |

我只做过Sub-I，没有intern的经验，不过医学都大同小异吧，也和大家分享一下。
每天给药的时间是固定的，我们这里BID是900和1500，Q12h是900和2100,TID是600，
1200，1800，Q8h是600，1400，2200。如果上午十点写一个抗生素Q12h的医嘱，护士要
到晚上9点才会给药，所以要在后面写一个first dose now，病人才会得到及时治疗。
不写的话主治医生下午一看，病人高烧半天了，什么抗生素都没上，肯定要皱眉头。
每个人都会犯错误，客气的指出别人的错误，避免医疗过失，别人会非常感谢你。我曾
经指出过一个chief忘记给术后的heparin，另一个chief医嘱写错写到别人的chart上。
还有高烧病人送了血培养实验室却返回尿培养，intern只打了个电话说实验室没收到就
放弃了，我反正也闲的没事直接到实验室去翻一张张送检单，结果发现是和另一个病人
标签贴反了。所以那些高年都对我特别好，因为他们知道我会仔细的检查每一个医嘱，
让他们很放心。

就算牺牲privacy, 来写个总回顾。感慨抒情以下先，那是多么难忘的第一次住院经历
啊。 生个宝贝真是九死一生， 一点都不夸张哦。
Chapter 1: 预产期前1天17号，好像她知道我终于不上班， 可以迫不及待的出来了
。 先是出血，出现宫缩， 开始进入labor stage 1 (latent phase)。好疼啊，从来没
有过痛经（考点：prostaglandin, tx NSAIDs）, 对这种痛实在不能忍受。去医院，医
学院附属，有医学生，intern, resident, attending。 告知宫开一指， ok, 回家 （
考点： 宫开 3-4指收入院，上epidural 也是宫开4指； 另外，stage 1, active
phase, cervical dilated>=3cm）。
Chapter 2: 好不容易过了一个晚上，第二天再上医院，该收我了吧，疼得实在不行
了。 还是1-2指。还好是我的OB on call，在我的要求下，终于收治入院 （考点：病

另外不知大家写完常规医嘱后，如果到了写patient counseling 的时候，又不出5分钟
医嘱的话，是先写，还是等5分钟医嘱出现后再写。因为如果先写，得一个一个打进去
