After that, you should be negative if you don't have latent TB or had no new
infection. That explains your situation.

发帖数: 442 | 4 在san diego，找的医生，一人一百，包括tb和血检。不过疫苗比较贵，85一针，hepB
要三针。但文件是打完第一针就给发了。公司报销就无所谓了。
另外疫苗可以找家庭医生，走保险，所以不用花钱。 |

但在体检表的HepB抗体检测一栏提到
下列人员要检查并付结果
1 怀孕妇女
2 澳居民领养的孩子
3 无陪伴的少数民族(?unaccompanied minor)难民孩子
4 有临床特征的人
相对HIV检测明确要求移民进行检查,可见对肝炎是相对松的 |

first line anti-vrial therapy for HepB because of its side effects,
inconvenience to take and the availability of many other oral meds with mild
side effects like, Lamivudine, Adefovir, Tenofovir, Entecavir ect. |

- if someone needs marked for a para/thoracentesis, order the abdominal
fluid survey (ultrasound) ASAP or do yourself with a senior with you
- Prior to procedures, check coags; does pt need reversed with FFP? do they
need platelets? may need to time it to start at 4am or 6am for morning
procedures (colonoscopy, EGD, etc)
- keep a close eye on Na levels and K levels given diuresis and fluid
overload in these patients

- Volume: be careful not to fluid overload these pts; hypoalbuminemic
already, 3rd spacing. If unstable, hypotensive, consider IV albumin rather
than NS. Can do salt-poor 12.5gm BID for a few days (not much data supports
use past albumin levels of 3.0); useful in renal failure to reperfuse
kidneys. Esp if concern for hepatorenal syndrome, can start albumin. If
bolus fluids, do so gently.

动作也就快了。医院里很多事情，看不过眼，下了班，又和我没有关系。现在有点体会
到老护士常说得，做事情之前，都要想到保护自己的执照。科里hiv,hepb,hepC病人多
了，我现在就把每个病人当作一样，really universal precaution to everyone.
我们这家医院规模还算大，离家很远，当时选它，就想着长点经验，过了几年，去哪里
都可以了。
工作上也尽量多做少抱怨，也发现科里抱怨最多的护士，也是做的最少，最难合作的护
士。

and Rotateq. 皮肤表面没有红肿， 只是有些发青，皮下有一个很硬的结。昨天给护士
打电话了， 被告知可能是皮下出血需要reabsorb. 让我过一个星期如果没有变小再去
检查。今天我自己给宝宝摸了一下，似乎是小一些了。 |

了，至少我们那个年代是这样的。或是体内都有了抗体。你需要做的是，第一，找个家
庭医生，告诉他学校或单位的要求，第二，可以要求测MMR和VARICILLA 的 TITER，如
果不确定HEPB， 也可以测一下抗体。 如果抗体都是阳性，你就不需要打了， 把记录
交给学校或单位。 哪个是阴性的则需要补哪个。 一般家庭医生那里提供 HEP B （三
针）。 但是一般没有MMR 或 VARICILLA 给成年人，HEALTH DEPARTMENT， WALGREEN,
CVS 都有，给保险公司打电话，可能报销。 一般单位还要求TDAP, 家庭医生有，直接
打一针吧，管十年。 |

知道中了咋办的。我有一天看了个新病人，看起来气色很好，病历也干净得很，结果
坐下跟我说，她pneumonia十几年，因为染了非常罕见的一种细菌，全美也就几千
个病例，她也不知道怎么染上的，花了十年看了很多医生才得到确诊 (所以自带的
病历上没纪录)，确诊后三年吃了三种抗生素，至今验血发现还没消除，肺炎每年犯
好几次，今天也正在犯，只是现在正好没咳嗽、、尼玛，你make appointment的
时候怎么不早说、、我强做镇定挤出笑容啊，该握的手也握了、、坑爹的是回头查
遍资料也查不到这细菌如何传染的，只好自求多福了、、 |
