脉，还有查心肺腹； 还有pass out的病人，也要查眼底，听诊心脏和颈动脉，还要查
全套的神经。这些好像不可能在4分钟内全查完，而且问诊已经有好多问题了，也省不
出多少时间，请问各位高人有什么经验么？多谢了。 |

0. 0.1 Basic rule1: be ambitious but also be practical, hehe.
0.2 Basic rule2: pick something that really worries you.
1. 好好享受最后的日子，多陪陪家人。
2. 好好放松加好好提高EQ:)
3. Intership开始以后你就知道睡觉不够是最大的问题。现在睡足可以至少扛一个
月。
4. 读书 + 听书
5. 练习英语， 说 第一印象很重要。
6. 然后练习 CASE PRESENTATION。
7. 复习一下CS，以前是为了应付考试，现在是真的要上阵了。
8. 让chief给你安排个好senior, 第一个月好senior能让你顺利过度，不好的
会让你痛不欲生。

4.1 就按如下几个病种复习好了(我内科有点忘了，大家再补充其他常见病种吧)：

70岁男性胸痛，有DM,HTN病史，EKG有多导联的2mm STD，刚好他一个星期前有个EKG是
正常的。serial TNT是阴性的，难道你们会让他先去做stress test?
这个是high risk for unstable angina,在这里的处理肯定是ASA,plavix,beta-
blocker,lovenox,该给的药都给了然后cardiologist会schedule cath. 一般是48小时
内，但也有人说越早越好的。
如果有这里的cardiologist澄清一下就好了。

Rejected了——急诊ST-T改变 + 心前区不适，可以数出不下20种情况，CAG是最潇洒的
确定诊办法，PCI

——这种情况,凭病史加ECG诊断就已经明确，当然无须stress test。
对这个病人，中美两国的医生，处理上都差不多。
经短暂的强力抗血小板治疗后，这个患者做CAG,已经不是诊断问题，而是明确病变的部位、性质、血管条件等，以决定下一步的治疗.......
冠心病，尽可能的、及时的血运重建；STEMI,充分、及时、持续的IRA开通，是大原则。但，个别情况，动不动就直接以CAG作为诊断手段，不对。
我也没有抬扛的意思，可能有些相互理解上的问题。 |

hysterectomy are all distracors. It feels like that the question actually is
asking about oral administration vs parenteral administration. Since the
other choices are all orally administrated so the answer is C? I am not 100%

，也给今年面试的同学做个参考。
1. Patient population
贫困区的病人往往年轻些，20-50岁居多。因为大多没有保险，重病人多是因为平时得
不到适当的医疗所导致。AIDS/吸毒病人比较多，另外也经常见到Asthma exacerbation
, Sickle cell crisis, 严重感染等。Resident的自主性比较强，procedure比较hands
on一些。气管插管，腰穿等等很快就certify了。不过抽血等scutwork往往也偏多。这
里的病人往往不太按时随诊，相应而讲纵向观察病情演变的机会略少。
富人区的医院病人往往年龄偏大，70-80很常见。病人除了入院的主要病情, 既往史往
往一大列，HTN/DM/CAD/CKD...单个病人花费的时间长一些。这里的scutwork相对比较

do. The prognosis depends on where is the stroke and size. The doctors will
decide whether your father needs to admitted to ICU. Fever and HTN are
common post stroke. Once your father is stabilized, a stroke team in the

It’s hard in the beginning, kind of clueless for me. But you will be able
to get the idea pretty quickly after 5-6 cases in computer. Here’s my
thoughts how to manage it quickly and try not to miss things.
1. Differential if need urgent care.
All Chest pain, SOB, AMS, post-trauma, little infant all need to be ordered
with ER order before physical exam.

good by to USMLE
Material: MTB3 and UptoDate. Step 1/Step2: 250/249 but those were 3 years
ago. Step 3: 227. Time: on/off for 3 months.
Good luck to everybody.
Not a whole lot of thoughts about this test. if you can memorize MTB3 and UW
, then there should be NO problem to pass it.

找到very very strong reason for doing this USMLE. Preparing for Step 2 CK is
like preparing for a marathon.
Start with a motivation: Motivation and mental preparation are critical in

history of Dm and 2y history of diabetic nephropathy. She had laser
treatments 1 y ago for proliferative retinitis with some loss of vision. She
currently takes intermediate and short-acting insulin before breakfast and
dinner. She also takes enalapril for HTN. She does home glucose monitoring

attending. Look at the big picture. (10 max for intern)
Actively looking for interesting cases, common mistakes, to be independent,
ask questions.
Respect nurse. Call and remind them if you add a lab.
Mistakes are common, if other people remind you, or vice versa, try to
maintain you and your colleague’s authority in front of pts, making
everyone happy.
