antiviral medicines such as
adefovir which has advantage against lamivudine-resistent strains of hep b;
entecavir, telbivudine,tenofovir are other antiviral drugs,
there are some anti-viral drugs in trials, include emtricitabine,
famciclovir, clevudine, val-d-cytosine and valtorcitabine, alamifovir,
pradefovir, i didn't read into the details andnot sure if you could get

有多少人真的得了Ebola），其次他的说法也很矛盾。下面是CNN的具体报道翻译版
CNN一篇报道可谓风生水起，利比里亚一名医生用治疗HBV和HIV的药物——拉米夫定治
疗埃博拉病毒感染病人，取得了很好的效果。按照新闻的描述，共有15名病人接受了治
疗，其中13名存活，2名死亡，这样算下来病死率在13.3%。而按照按照官方公布的数字
，这次埃博拉暴发的病死率在50%多一点。
一种药物把埃博拉的病死率降低了那么多，不是很不错吗？这位医生说，这13名活下来
的病人，都是在发病前5天内使用拉米夫定，而那两名不幸死亡的病人，都是在地5天和
第8天之间才使用该药物。言下之意就是，如果在发病早期使用拉米夫定治疗，那么效
果应该会很好。
这位医生说，它是看到了学术论文中提到埃博拉病毒和HIV在细胞内的复制过程很相似

AST)都超出正常范围，表面抗原(HBsAg)和e抗体(Anti-HBe)阳性，乙肝病毒DNA含量4*
10^6,显示病毒复制活动。心中焦虑。有两个问题诚心求教各位有经验的兄弟姐妹：
1. 我年纪不小了，正打算要小孩子，没想到节骨眼上来了这个问题捣乱。请问目前我
的情况，是不是要赶紧治疗，这两年不能考虑怀孕了？
2. 我在美国见医生的感觉不太好，家庭医生根本不懂那些指标。请问是否有必要回国
治疗？但似乎网上资料显示国内的治疗方法也与美国大同小异，不外乎抗病毒药如
lamivudine(拉米夫定）和adefovir,以及干扰素（interferon）。请问国内还有比较有
效的办法吗？

发帖数: 117 | 19 前面有人提到‘努力吃药’，这里努力吃药的意思是。。。虽然没有药物可以杀死HBV
病毒,但是可以通过一些抗病毒核苷类药物或者干扰素来抑制病毒的复制，降低病毒在
人体的活性，有名的有 lamivudine (拉米夫定), adefovir （阿德福林） 和
entecavir （恩替卡韦）, 以及 telbivudine. 一旦有需要开始服药，就是长期性的，
所以要‘努力’。

很相似，所以他得到灵感使用抗HIV药物来尝试治疗埃博拉病毒感染

“CTL可以在DNA数量低于10的7次方产生”（ref见下）
临床上相当多病人ALT反复升高，但是无法清除HBV，显然CTL对于清除HBV不是万能的

first line anti-vrial therapy for HepB because of its side effects,
inconvenience to take and the availability of many other oral meds with mild

B, liver function test is abnormal ( the case is almost same as your friend
). The doctor in China provide him IFN and his WBC drop to below 500. then
stop it, he only use entacavir, after only three month his viral load start
to decrease and 6 month later his viral load is less than 1000. his Liver

Step3考场壮烈回来。第一天挺累人，最后一个block甚至做到有心无力状，看着题反应

复习资料：
UW 52 online cases : 基础。最好复习早起找高手一起过一遍，尽快摸熟软件，进入
状态。第二遍找一位水平相近童鞋过一遍，不断总结protocol. 考前迅速做一遍，熟练
运用protocol.
UW 41 offline cases: 我当时和partner每个case仔细做一遍，虽然不像online有反馈
，但对练临床思维，补充protocol非常有帮助。最后考试与41 case有异曲同工之处，
遇到复杂case也不慌神。
