我填的surname和last name都是我ld的名字，和之前的几页一样，可是在这页非得说名
字有错，不能往下走了。我看上面说是要include maiden name，应该还这个无关吧？
我们中国人都是没有改过姓的呀。
而且前面也有让填主申请人的名字的地方，根本没问题，
不知道为什么卡在这里走不了了？

的日期（2011年）而不是去年回国归来的日期（2012年）。

写A，时间写得有重叠也没有关系吗？ |

1) Since I used my Advance Parole to re-enter last year, in Line 14 of I-
765, should I put "Advance Parole" as my "Manner of last entry" ?
2) In previous renewals, my lawyer filed with the Texas Service Center (
where my I-485 is pending). According to the current I-765 instruction, I'm
supposed to file with the USCIS Phoenix Lockbox. Should I follow this or
stick to the Texas center as before?
I am wondering if somebody knows the answers. |

on the passport without any other record. Should I use the date that I left
Canada and entered U.S. as "last entry" or use the date that can be found on
my passport (back in 2006)?
