do is to compare it with other CXRs. Duration is very important.
Other points:
if the border is clear, if not, invasive.
if calcification around nodule, if yes, malignancy.
if lymphadenopathy, if yes, cancer.
if smoking history +, if family history +.
if there is any endocrinolgoical changes recently, if yes, pancoast tumor.
Many others.
If malignancy is more likely (80-90%), go for thoracotomy.

malignancy in the future, so surgery is indicated.

I concern about "ulcerated mass" and the size of the mass. The path report
did not idenify malignant cells but as you know, the biopsy could miss the
cancer tissue. As mentioned in the comment of both endoscopy and CT scan
report, most likely this is cancer, and it causes symptoms. The next step

7月份b超是这样的，大家看看像不像恶性? 甲状腺双侧叶及峡部大小形体正常，被膜光
滑，腺体组织回声欠均匀，稍增粗，甲状腺左叶内见多处大小不等低等回声结节，较大
者位于腺体浅层，大小2.4*1.0cm,边界清晰。右叶中部深面似见0.7*0.4等回声结节，
边界欠清晰,CDFI上述结节周边可采集散在血流信号。左颌下见1.7*0.8淋巴结回声，左
颌下见2.1*0.9淋巴结回声。
8月份在国内另一家做的，想看一下那个大的变大没，没想到不同医院仪器不一样，没
法比较：甲状腺右叶1.79cm,峡部厚0.32cm,左叶厚1.89cm,实质回声增粗欠均，甲状腺
左叶可见中等回声结节，大小约2.36*1.15cm,边界尚清CDFI:期内可见血流信号显示，

malignant unless medullary thyca is suspected. also biopsy should be
performed, however, depending on the location of the lesion it could be
difficult to biopsy given its size of 3mm. the patient could also choose to
watch and wait given the slow growing nature of thyca. i also saw an
article that says 70 percent of calcified thyroid lesions are malignant.

specialist. specialist 说 malign 的可能性不大，因为可以动啊。最大可能是

Malignant tumor equals to cancer. Benign tumor is very common, and usually
is not fatal unless the tumor grows in unusual places. For example, benign
brain tumors could be fatal. This is because the space within skull is very
limited. When the benign tumor grows bigger and bigger, it will push other
brain parts. So its not the tumor itself kills patient but the "compression
effect" of the tumor causes the trouble. |

今天的主要活动是Athabasca River rafting (Heritage Run- 3 hrs) 。
这是俺们有生以来的第一次 漂流。所以挑了个最容易的，class 2。
即使这样，怕水的老公对这个活动仍颇有微词，偏偏天气又不好，报完到，
刚换好游泳衣，倾盆大雨就浇了下来。回头看看老公，已经是满脸舍命陪
君子的悲壮。两个帅帅的小导游，一个劲安慰大家不要担心，雨太大,
天气太冷的话他们会缩短行程的。
旅游公司的汽车把一群老老少少拉到了漂流的起点，雨已经停了，这就是
山里天气的特点。变幻莫测，后来的几天我们对此有了更深的体会。
换上wet suite，大家被分成两组，一组全是带小孩的，我们这一组是三对夫妇。
我们， 一对老夫妇从英国来，一对年轻夫妇从南边过来。我们的导游叫Steve，
让大家自己选择做哪里，年轻夫妇当仁不让的选了第一排，英国老夫妇选了第

发帖数: 27 | 19 下午去Malign Lake，事先定了5点钟的boat tour，就是为了去看看久负
盛名的Spirit Island. Malign Lake一整天都笼罩在乌云中，上了船，
四周一片烟雨凄迷。
导游小伙开始了讲解，语调既不跌荡起伏，也不抑扬顿挫，俺开始迷迷
糊糊的打起盹来。刚刚盹着，就被老公用胳膊纣叫醒了，老公说，快听
听，小伙子从名人八卦讲到野生动物保护，信息量很大啊。俺支着耳朵
勉强听了两句，听到他说某个名人，先嫁个老公大她20岁，后嫁个老公
小她20岁。不错啊，俺心里嘀咕一句，又见周公去了。一直到有人开门
到甲板上去，俺才被冷风吹醒。听到导游紧着告诉大家，甲板上最多只
能站8个人，不然船会翻的。扭头一看，老公已经跑到甲板上去了，赶紧
数数甲板上的人数，还好，只有6个。俺一直百思不解，这船也装了几十号
人，咋就这么脆弱呢？
到了Spirit Island，小的让人有些失望，看不出任何特色。也许是因为
天气不好的缘故，看不出那张让它盛名远扬的照片描绘的境界。导游给了
大家十分钟，沿着湖边的一个小路转一圈，拍两张照片，等天 好的时候，
再重游此地吧。

（八）Jasper浅尝辄止
Jasper公园很大，trail也很多。我们时间有限，只能挑几个地方看看。一大早，我们
开车上了Jasper Town东面和16号路交接的一条公路。第一站是Maligne Canyon。这个
Canyon和前几天看到的瀑布峡谷比较象，基本上是沿途看河水冲刷形成的峡谷。河水还
是那种带点浑浊淡淡的绿色。峡谷上架着六座桥，从一号到六号桥要3.7公里，也可以
不走trail直接开车到二号桥或者五号桥边的parking lot。整条trail有些上上下下，
但总的比较容易，而且风景也很好，能看见激流，沿山而下的瀑布，地下暗河，溪涧中
的小岛，以及河中间冒出来的泉水。如果早点走，trail上没什么人，效果更好。我们
从一号到五号桥走了个来回，迷迷糊糊的忘了还有六号桥这段路没走，想想也就算了。
接着我们要开车到这段公路的尽头Maligne Lake 处。途中又看见一群big horn sheep
在马路中间散步，看见车过来，不但不怕，还把头凑上来盯着车里望，呵呵。接下来路

Day1：晚上11：30飞抵Calgary机场，过关约半小时，取车，住在离机场10分钟车程的
super8
Day2：在附近超市买东西后直接开往Jasper，住在Maligne Lodge，安顿好后到
Medicine Lake看日落，路上人烟稀少，遇到elk数只。
Day3：清晨6：30，去附近的PYRAMID LAKE拍日出，中午去MT EDITH CAVELL，Tonquin
Valley冰川和湖很美，遇到白马饮水于碧湖。晚上6点半再开回MALIGNE LAKE ROAD，拍
日落，再见elk数只。
Day4：清晨出发，走93A （和93平行，一过检票口200米右拐就是），路上有瀑布和安
静的湖泊。中午时分到达WILCOX PASS TRAIL，走了trail，很带劲儿，走到山顶，狂风
大作，冰雹袭击。附近有Columbia Ice field，朋友建议不去。下午到了BOW LAKE边上
的NUM-TI-JAH客栈，很美的地方，走了TRAIL。继续走，晚上住在Lake Louise Inn

准备工作：
签证和身份问题：有美国签证签加拿大非常简单，也不需要面签，可以把材料寄过去签
。我在美国还是学生签证，已毕业，在实习，OPT F1，美国签证过期，有合法I-20，很
顺利拿到加拿大签证。注意：有合法I-20表格的人，即使美国签证过期，去加拿大，墨
西哥，加勒比地区30天以内，不需要美国签证依然可以回美国（注意：要享受这种政策
上等于没有离开过美国的待遇，要保存原始I-94表，出关的时候不要被收走）。在实习
的同学需要准备工卡和工作证明信。OPT期间可以大胆放心旅行。我回来从5号高速公路
入美国境，非常顺利，没有问我要OPT的工作证明信，只问我在哪儿工作就过了。工卡
也不看。整个过程不到一分钟。没有换I-94。等于说我们并没有离开过美国。

先在Edmonton去了Elk Island National Park.因为到傍晚了，看到了一大群bison.
后来去Jasper：
Maligne Lake够大，漂亮；
Mt. Edith Cavell ，推荐！
Maligne Canyon, 走trail,小累。
Columbia Icefield的tour,我们赶上了5：30pm倒数地第二班车，这个时间去真是太好
了，只有10个人！
再后来去了Banff：
Sulfur mountain, 坐缆车上去，7分钟，走下来，1个半小时。这是我们走的最长的一
路，还全程往下冲，结果我和LD第二天都腿疼，孩子们则一点事都没有。这一路还是挺
有趣的，拐来拐去的，时不时从缆车下穿过，经常能鸟看一眼Banff全城，树上蓝鸟叫
红松鼠跳。。。
Bow river的raft tour,很平缓，很放松，有点象武夷山的竹筏漂流tour的感觉。
下了船来，小朋友们又在岸边扔了两个小时石头，其乐无穷，似乎比看山看湖有思多了！

发帖数: 1542 | 27 去lake maligne路上路过的lake medicine挺美的，lake maligne是比较安静，尤其是
环湖那个小trail
lake o'hara是那边一带最美的地方，就去了opabin和oasa两条路就特别特别美了，但
都得靠走。估计那个大环的alpine circut更美吧，只是去的时候太早那条路有雪不推
荐走。emerald要等快日落的时候，湖水很安静。
lake louise就是生活方便吧，没走six galcier那个trail，就走了lake agnes那个，
再上了little beehive去俯瞰了lake louise，就像城堡酒店的游泳池一样，看bow
valley的景色也很壮观。
如果选肯定是Lake Moraine >> lake Louise，去划了一小时船太美了，如果有时间一
定要走一下larch valley那个trail，上面景色很赞！

一路上像松鼠这样的小动物遇到很多， 而且是近距离接触。 在去Edith Glacier的路
上遇到了一只熊妈妈带着两个熊宝宝在路边觅食。 一溜车都停在路边狂拍， 但都不敢
走太近， 生怕激怒了熊妈妈， 把我的胖儿子抓去给熊宝宝做晚餐就太悲剧了。
在Maligne Canyon两次遇到鹿群。 鹿子很敏感， 一举起相机它就警觉的抬起头， 随
即退后两步。 在去Maligne Lake的路上遇到的山羊则是毫不害怕， 还把头伸进车里来
东张西望， 反而把领导吓得尖叫， 而两个孩子则兴奋的大叫：“妈妈， 快照相！妈
妈， 快照相！” |

第一天中午从PHX 直飞YYC，住Four Points by Sheraton at Calgary Airport. 在大
统华随便买了些吃。
第二天前往Banff, 先去Cascade Pond, 然后是Lake Minnewanka, Two Jack Lake,
Johnson Lake， 围着Johnson Lake走了一圈， 然后是Tunnel Mountain, Gondola上
Sulphur Mountain, Hot Springs, 晚上歇Best Western.
第三天走Bow Valley Parkway, Johnason Canyon, 一直走到Upper Springs. 然后去
Verzillion Lakes, 结果发现这是此行最坑爹的景点，完全是浪费时间。惺惺然的回到
Banff 镇上，参观了Cascade Garden, Bow Falls, 走了镇附近的两个trail, 继续住

我查google是两个小时的车程，但看攻略上从Banff镇过去就要两个小时。是不是把去

如果接受器官移植的人用药导致不排斥异体器官,也就会同样的允许了可能不小心移植
来的肿瘤细胞的繁殖.
角膜没有血管,但周围的边缘limbus是可能出现转移的癌症的.而手术中不太可能保证一
个潜在肿瘤细胞的污染都没有吧.

也发生过癌症患者去世后捐献角膜,后来病理检查剩余组织发现有癌变,导致接受角膜移
植的患者要接受再次角膜移植, 来替换掉第一次移植的有癌变可能的角膜.

1. 10-2003 确诊，07-2004终于手术切除，期间Jobs一直在试自己选的diet疗法。扫描发现癌变在扩大才动了手术，这段时间癌转移的多少不可知，但肯定越拖转移的越多。
2.手术后没化疗和放疗。Jobs 2005年演讲说这表明病情乐观，不知是哪个庸医给他灌输的。2004年手术是大面积切除，部分胰腺和胃和小肠，胆和胆管都切除，目的是要消除扩散，但不系统化疗和放疗怎么能最大程度减少扩散威胁？哪个外科大夫这么信自己的手艺？
3.2009年换肝，这在扩散性癌治疗是有争议的，因为换肝后压制免疫反应有可能促进别处癌的生长。也有可能当时肝已经不行了没办法。
这种癌正常治疗大多数可活10年以上，Jobs真该到东岸Hopkins或Harvard找个靠谱的大夫。

2. what cell type in pancreas express this gene, epithelial or other?
immuno staining should answer this question.

transient transfection, cell viability assay, tumorigenecity (in vitro and

我们可以这么分析
Rituximab 治疗 B cell lymphoma 基本上是神药了～ 这么多年来，genetech挣了不少
钱。 其实机制很简单，就是把病人体内 normal和malignant的B cell全部杀光，
targeting CD20.
Car T 目前最好的trial是 target CD19, 跟CD20机制类似，说到底就是 换汤不换药，
用transduced engineered T cell把CD19+的normal 和malignant B 细胞一起杀掉。目
前 clinical trial主要用于 B cell leukemia.
其实rituximab治疗B cell leukemia的效果很好，你去查一下clinical trial的结果，
naive patient 90% 的有效率～ 但是，pharma也没有很积极的把rituxan推向clinical
trial。原因是“rituxan现在很能挣钱，就让它继续挣，专利15年过期，再用于
leukemia挣钱” 或者“用car 挣钱？“
