发帖数: 4747 | 2 我偏头痛好多好多年了，以前在国内必理通加合百服宁什么的都吃过，正天丸也吃过但
是没坚持。不管吃什么药，最后在我身上都没效果了。
来美国后，网上的人都推荐excedrin migraine，我买了，非常好用，好用的有点让人
害怕。说明上说成人一次两片，起初我是老老实实吃两片的，发现这么好用之后我就改
吃一片，通常一半的药量1小时内就见效！我相信所有的止痛药都是麻痹神经让你没有
痛感，并不是真的在“治疗”头痛，所以我有两个担心：
1 总吃会不会就产生抗药了？以后吃了不止疼怎么办？
2 吃了那么快就见效的药，是不是太厉害了点？一疼就吃，会不会对身体也不好？
我现在能忍着就不吃，能睡着也不吃，实在难受了，或者影响我做别的事情了，才会吃
。这瓶药我大概是10年9，10月间买的，100粒的药，还剩19粒，中间给同事大概4粒，
我自己两年半吃了77粒。这瓶药下个月就过期了，大家有没有别的药推荐的？是不是换
别的药吃一吃不太容易产生抗药性呢？如果没有其他的推荐我就还是继续买这个药吧。 |

从第一次发作到现在，看书或者电脑的时候，行间和空白的地方一直闪，神经科的医生
说是migraine的症状，眼科的医生说和眼睛没关系，但是绝对不是migraine，因为

If it is minor, bubble bath can help. Then sleep. Then over the counter pain
killers with sleep aid. And prescription migraine medicine as last resort (
it will knock you out for a couple of hours).
There are a lot of triggers for migraine. Food can be one (which we can
control)

你见过几个neurologist给migraine病人order EMG的？ migraine有时可以出现stroke
症状或者seizure， migraine与中风和癫痫的关系你应该很清楚， 某些病人需要MRI和
EEG， 但绝对不是常规。
退一万步讲，即使你没有职业道德做一些不该做的检查，不说保险公司给不给
reimbursement，referring美国医生也不是傻瓜，病人也不是傻子，几次下来，名声坏
了，你还想不想混了？
当然这在VA肯定不是问题。 ：） |

This is a real case. MRI is defensive medicine and I was compromised on that
. This case has Right CN2 involvement as well as Left CN5 and right CN3
symptoms. The latter is persistent so the patient will have diplopia for a
while. Where is the localization? Brain, of course.
I was joking with the ER Doc that CN2 + CN5 + CN3= CN10. There is no such a
disease. Stroke? maybe, but MRI is negative. And I have never seen a stroke
with pure pupillary sparing CN 3 palsy.

动一样的疼。医生检查排除耳朵感
染，strep throat，和mononucleosis（一种青壮年益发的病毒感染）。服用
antibiotics和止疼药一周，病情没有减缓。
开始疼后第四天发烧，但不是很高，98－100F，但从此人感觉fatigue。现在不知是否
因为止疼药吃多的原因，右半边脸
麻木，右眼不能灵活眨眼，对光敏感，流泪。右脸肿，且嘴巴往左歪，吃东西也不利索
。一停止疼药，疼痛感加剧。
刚开始以为是migraine，但医生说不象，而且家里没有人有migraine病史。小于35岁，
应该不会是中风吧，且嘴歪脸木

我有个同事,20出头,白女,从很小的时候就有migraine,平时总是喝可乐咖啡什么的,看
过好多医生,但是一直都被当成migraine治疗,吃过各种各样的止疼药.前年有一天半夜
突然发生seizure,然后第二天去医院坚决要求医生给做了MRI,发现脑子里有个肿瘤...
所以如果长期头痛,不是小事,一定要重视. |

前一段时间去见精神科医生，认为可能是抑郁症，remeron and ativan as needed. 症
状时好时重。自己上网了解，自我诊断为 Migraine Associated Vertigo (MAV)，不知
道有人了解吗？另外，这两种药Nortriptyline 和remeron 可以同时口服吗？谢谢大家！

似的一群病的相关症状?比如headache,如果你怀疑migraine, 你是只问migraine,还是
所有的头痛相关疾病你都问(比如infection, pseudotumor等等)?
因为我现在就是问症状相似的一群病的症状,比如说,如果headache我就不问不相关的症
状(比如cough或abdominal pain),只问7-8种可以引起headache的疾病.还是除了differential之外,还要问主要的ROS(from head to toe)?这样势必存在时间不够的问题.看了你们的讨论,比较困惑,所以希望澄清一下.各位pass的朋友也可以补充一下.先谢谢了.

看到一个头疼
的年轻女病人，问你有哪几个可能？就答说 acute migraine, cluster headache,
tension headache, etc. 可能追问一下 acute migraine和什么有关？答和她的月经有
关。
我觉得attending多半考问的还是medical school level的问题，刚开始的同学要在这
个方面
下工夫。答好了，attending会慢慢加到resident, fellow level的问题。 |

aphasia is from ischemia of left middle cerebral artery. Ataxia, nausea and sluured speech are from basilar artery.
To put them togather, only migraine makes sense to me.
Food poisoning is another possibility but i doubt it.. |

treat chronic daily headche or transformed migraine. But how to promote the
use of botox is another story.
Off-label use of Botox for migraine is difficult due to the price. FDA
approval makes it affordable to those ordinary patients. |
