push了,朋友说疼得不行,epidirual再加量根本没用.她老公问她前面2个生的时候有那
么疼么,她说没有那么疼.反正后来哗啦啦全出来了.老三接近8lb,是个大宝宝.然后医生
就在缝针,宝爸抱着宝宝在旁边看着,挺高兴.

还会有，不知道要不要做羊穿……好紧张。求祝福……

Update:
从high risk obstetrical consultant 回来了，又做了一遍B超，医生说这个小白点实
在是没有必要担心，只是把我的唐氏概率从1：850变成了1:(850/1.1)，而且亚裔本身
有白点的概率就高。还说要是我实在是不放心可以做quad test或者NIPT,前者保险包括
，测血液里的荷尔蒙指标，后者out of pocket maximum $150，测血液里的婴儿DNA，
准确性更高。于是我选择了后者，抽了两管血，孩儿他爹也史无前例的参与了进来，特
别兴奋，拿棉签在嘴巴里蹭了半分钟。两周后出结果，如果正常，就可以把概率降到1
：5000什么的，如果有问题，那就再进一步做检查。然后我们就从医院兴高采烈的出

看疑难病症。我的医生就是。她的title 是 Perinatologist, board certified in
both Obstetrics and Gynecology and Maternal-Fetal Medicine. 我觉得比一般冠有
high risk 的 ob 专业多了。

发帖数: 1519 | 24 上来就质疑midwife是否知道胎儿窘迫，说明你对这个行业的无知。你对你不知道的东
西这么反对，我就不说你什么了。
我给的建议完全是合理的。剖腹产率15-20%的国家中，很多是大量使用midwife的欧洲
国家，他们的母婴死亡率和美国相似，也没听说过欧洲的母婴发病率比美国更高吧？
ACOG在2014年3月发表文章（下面有链接），安全避免主要剖腹产。美国从1996年到
2011年剖腹产率迅速上升，同时却并没有产妇新生儿发病率死亡率的显著下降。因而指
出了现在剖腹产有过度使用的问题。
美国现在全国的剖腹产率33%，各州不一样，从23%到将近40%，具体到各个医院，则从7
.1%到69.9%都有，这可是10倍差别。即使只考虑低风险的产妇，剖腹产率则从2.4%到36
.5%，15倍差别，年龄，体重，种族等区别都不能完全解释这么大的差别。如果你是一
个健康产妇想要顺产，认真选择一个剖腹产率低医院和caregive完全是合理的建议。大
部分产妇都可以顺产，有OB后备的midwife是一个理智的选择。好的midwife有几千生产
经历，你要说有的midwife水平不高，那我只好说OB也有很差的。

1.催产不会增加剖腹产率
1990年催产率是9.5%，2008年催产率是23.1%。确实催产产妇比已经自然发动的产妇剖
腹产率高，但是和继续等的产妇比起来，足月催产并不会增加剖腹产率，甚至降低剖腹
产率。特别是过预产期一周以上的产妇，催产可以降低母婴风险。促进宫颈成熟的药也
貌似安全，不会增加剖腹产率。

2.在某些国家（非美国），midwife持续关怀和标准医学关怀的剖腹产率相似

）和英国（22.0%剖），midwife持续关怀最终剖腹产率和标准医学关怀的剖腹产率相似
。总体婴儿死亡率没有区别（这两组产妇是随机安排的，各种风险都有）。

我因为娃偏小被安排了下周二催产，OB说因为我现在还基本没开指，会先上一个促开指
的药，叫作misoprostol（商品名是Cytotec），等开指到一定程度再上pitocin，也就
是催产素。
结果我查了两种药物，这个叫作misoprostol的在FDA只批了可以抗肠胃溃疡的，没有批
催产的，而且在FDA网站上有这么一段话：
A major adverse effect of the obstetrical use of misoprostol is uterine
tachysystole which may progress to uterine tetany with marked impairment of
uteroplacental blood flow, uterine rupture (requiring surgical repair,
hysterectomy, and/or salpingo-oophorectomy), or amniotic fluid embolism and

研究指出，原來女性甫過30歲，分娩的風險已較25至29歲增加20%。
瑞 典卡羅琳學院與卑爾根大學合作的一項研究分析瑞典及挪威100萬名首次分娩的孕婦
的資料，對比30至34歲孕婦與25至29歲孕婦的分娩狀況，發現前者超 級早產（懷孕22
至31周分娩）甚至胎死腹中的機率，較後者高出20%；如果孕婦是吸煙人士或過胖，風
險更與35至39歲的孕婦無異。
領導研究的Ulla Waldenström教授指出，生物學上，首次分娩的最佳時期是20至
30歲。研究刊於《婦產科》（Obstetrics and Gynecology）期刊。

发生，心里一直忐忑不安，很想早点见医生，但只见到了campus health 的一个护士，

发帖数: 3008 | 49 如果妹妹打算在northwest community hospital生， 那么就到此医院网站上找到附属
的OB/gyn list， 然后到ratemds.com和www.advocatehealth.com/Obstetrics去查OB/
GYN的review好不好。 选个好的再到你的BCBS PPO网络里看这个ob是不是in-network。
ob office最好要么离家近要么离office近， 因为最后3个月， 检查频繁， 要是离得
远， 挺着大肚子开很长时间车也是很累人的， lg不可能每次产检都跟着去， 给你开
车。
ob office设备也挺重要的， 太旧的ultrasound出来的照片看不清楚， 要到很后面才
能看出男女， 机器新， 很早就能知道是男是女， 你可以早点买小孩东西， 有deal就
可以开始囤。
友情提醒一下， 在你考虑选男或女ob， 男OB手法好些， 内检不疼， 女ob手法差些，
内检会疼噢！但如果你很介意男ob检查， 那就选个女的。
另外你还得打电话去问ob office是不是去医院做接生的， 有的医生只做检查， 不去
