先介绍下我的情况，我是运动学 kinesiology 的硕士，今年刚刚上，但是本身对搞研
究并不敢兴趣。想考一个有执照的科目作为以后发展的职业，但是由于本科跟医学没有
关系是工程类的，所以很多执照要受到很大的限制。
这个信息是上 CLINICAL GAIT ANALYSIS的时候老师有提到过，我留意的。

orthotics）专业，但我只是完成了学业，还没有找到地方做一年实习（resident），
我现在在一家儿童医院(18岁以下）的假肢矫形器中心做志愿者(volunteer)。所以我只
能告诉你我所知道的。
在美国脊柱侧弯（scoliosis）要先由大夫诊断是什么引起的，然后由有执照的矫形师
（certificated orthotist）做矫形器， 然后是物理理疗师（physical therapy）辅
助病人做康复训练。矫形器的应用范围在25--45度，能恢复多少要看个人情况，女孩的
最佳矫正时间是来月经后的1年半以内。现在美国比较流行的矫形器是Boston Brace。
我不知道国内的矫形器行业是一种什么状况，在美国都是要经过一年专业培训在加上一
年的临床实习才有资格做矫形器。我觉得你朋友女儿的情况现在最好找个人做一个合适
的矫形器。

1） 我晚上睡觉很安静。但是牙医说我可能有磨牙的习惯，因为我的牙比同龄人磨损严
重。treatment plan建议做Occlusal Guard （D9940）。今天打算去做了，可是牙医的
plan又变了，说上次code输错了，其实应该做的是Occlusal Orthotic Device （
D7880）。当下觉得非常不高兴，所以没有继续。请问这两个treatment区别在哪里？如
果放任不管我的牙会有什么问题？
2） 牙医建议我后面3个大牙做Application of Desensitizing Resin（D9911）。这项
保险是不报销的。但是同样的Application of Desensitizing Medicament（D9910）只
要一点点copay。想知道这两个treatment区别在哪里？同样如果放任不管我的牙会有什
么问题？

night guard. Occlusal guard是用来治磨牙(bruxism)的，以免牙齿经常空磨被破坏。
Occlusal orthotic device是矫正颌关节的，如果你的上下牙不齐，造成颌关节或肌肉
需要额外用角度和力帮你正常上下咬合，时间长了，你的颌关节和肌肉会过度疲劳，严
重的可以引起痉挛或头痛。脱敏的resin是树脂，medicament是药膏，方式不同，使用
上因人而异。不在乎酸冷疼痛不舒服，就不做呗。 |

bone spurs. Fortunately, this is the exception rather than the rule. Most
cases can be resolved with a combination of icing, rest, foot stretches and
supporting the foot with an orthotic shoe insert specifically designed for
this condition.
Large spurs CAN cause pain.

是一个trend 他个人还是支持有保护的鞋子 保守一点
另外关于 需要矫正跑姿来保护膝盖等 他说那种需要的情况 可以考虑orthotics 然后

medications, injection of corticosteroids 都用过，基本没什么效果，一跑就疼，
后来只好彻底休息了三个月疼痛才消失。以后我就特别小心，慢慢在跑步机上恢复训练
，再慢慢加路跑，又有两三个月才恢复之前的量。减肥也很有帮助，我最近半年减了
30lb，虽然训练量加了不少，PF也没有再犯。 |

不要热敷. 前4-6天每天有空的时候就拿冰袋敷着.
BRACE用hinged knee brace比较有用. 就是可以让膝关节弯曲但不允许左右的动度.
这种BRACE一般要处方. 但你可以到你附近的ORTHOTIC/BRACE SHOP去看看. 或去之前打
电话问问要不要处方.
如果冷敷了近一星期还很痛而且不能正常行走, 我建议去看个骨科的运动专科.
韧带的拉伤是一种需要时间愈合的伤. 如果一两个月后还有疼痛感也不要着急. 韧带需
要几个月来愈合. 而且这几个月内也不要做刺激受伤部位的活动. 这样减小再次受伤的
可能性.
另外还有一个可能性是MPFL(medial patellofemoral ligament)的拉伤. 不知道你当时
受伤的详细动作是如何的. 如果是扭伤, 那MCL的可能性大些. 如果是直接撞击伤,
MPFL受伤可能性大些.
说了这些, 确定的诊断还是来自着手的体检. 我就你讲的也就只能说这么多.
但就这两个伤, 治疗的开始都是冷敷, 然后慢慢的低强度的渐渐恢复动度. 如果几星期
后还有相 |

shoes". It is individually adjusted shoe, which means not only the child
should be presented ( need to make a cast first), but also it needs rehab
doctor's prescription. As far as I know, it costs approx $300-400 out of
pocket money if you have a private insurance. Some rehab doctor may want to
give Botox injection to loose the muscle contraction caused by cerebral
palsy. |

候会有隐型的小脚趾长骨骨折。如果是plantar fasciitis还真没有根治的方法，不过
orthotics+steroid injection会减轻疼痛很多。

发帖数: 7 | 50 不是太同意这样的说法，当然我自己本身就是DPM第二年的医学生，所以有些偏见哈哈
。一些人对DPM的印象主要停留在20，30年前，那时候DPM主要就是做些足部的护理，一
些小手术和orthotics. 不过从2000初开始整个filed变化的很厉害，原来在80年代DPM
是不需要residency的，90年代开始出现1年，2年的residency. 现在是minimum 3
years，第一年主要是off service rotation，也就是internship, 是和
其他specialty一起做intern的。Residency做完后还可以继续进修fellowship，有不少
DPM还会去做MD Ortho的foot and ankle fellowship。所以说，2000年后毕业出来的
DPM在足科方面的教育是绝对优于MD/DO Ortho的。就拿我们学校来说，一些来讲课的
DPM会提到会接到Ortho的referral, 因为骨科医生认为自己没有经验和相关的训练做足
部方面的手术。毕竟我们从医学教育开始就接触lower extremity, residency在足部手
