were both elevated and troponin negative X1. He was chest pain free after
pain meds and nitro paste given. The second set enzymes showed the same

Generics != OTC
OTC Tylenol is still brand name. Acetaminophen is the generic name for
Tylenol.
We will never see the day when Plavix goes OTC. But its generic med will be 80%-90% cheaper once the drug is off patent. |

了啊。而且，为了吃aspirin而加吃proton inhibitor，或者为了吃波立维而做基因测

回复楼主，我虽然还没开始学医，不过正好对此略知一二，因为我爸吃aspirin眼角会
出血，所以一直吃波立维(Plavix)。但对大多数老人，长期吃baby aspirin是标准做法
，我亲见过这里的医生对年纪大的病人几乎一概建议吃baby aspirin。如果你妈以前一
直吃都正常，应该就不用担心usmle10说的溃疡的问题，因为副作用不会只限于胃，眼
角牙齿等很多地方都可能会有出血现象。 |

know, current clinic guideline has yet recommended genetic test per primary
care setting at the present time. Further studies are needed to determine if
such test is meaningful and cost-effective. Please update me if you have
more information on this, especially the data collected from non-Caucasian

70岁男性胸痛，有DM,HTN病史，EKG有多导联的2mm STD，刚好他一个星期前有个EKG是
正常的。serial TNT是阴性的，难道你们会让他先去做stress test?
这个是high risk for unstable angina,在这里的处理肯定是ASA,plavix,beta-
blocker,lovenox,该给的药都给了然后cardiologist会schedule cath. 一般是48小时
内，但也有人说越早越好的。
如果有这里的cardiologist澄清一下就好了。

intern首先做好的心理准备是You're nothing.千万不要产生我马上就是一个Dr.的飘飘
然的感觉，如果是这样，你的自尊心很容易被催跨。我觉得要适应住院医的生活，首先
要尽快适应美国的医疗系统。而social work这项，我们开始的时候最不适应的。
从收病人开始，首先要了解病人是否有保险，因为这影响你的discahrge plan和用药。
如果有保险，还要看是否是medicare,medicaid,因为现在政府赤字很严重，对medicare
,medicaid管的是越来越严。然后要了解病人是否有PCP,这跟出院后的follow up紧密相
关。再就是病人如果年纪大，出院后是否要把他送到老人院去。因为很多时候，家属觉
得无法在照顾他们的亲人，于是就把它们扔到了医院。最后是code status,这点我们刚
开始的时候也会不适应，多留心跟周围的senior学学。
了解病人情况后，马上要和social worker沟通，比如NH placement,home health,

看来intern 还真是得做点请杂工的活， 你说的一下的是都是我现在当护士忙活的。
如果病人没有保险，出院的用药也是问题，有机会把walmart 4 dollars list打出来。
当然有的时候SW也会帮忙，比如plavix,即便病人没有保险厂家也会免费提供。
你还会遇到病人赖在医院的，病人出院前一定确定好是否有人来接他，啥时候来接他，
如果有人借口没人接不出院，这又是sw的事情，他们会帮你安排ride的。我还曾经遇到
患者拒绝出院，因为家里没电

, Lovenox （Sandoz的generic已经上市，后面还有一堆排着，新一代口服的新药也都
在上市） , Taxotere （patent 被Hospira invalidated)....pipeline又是问题不断
。未来的几年，除了pfizer，就是aventis日子最难过，也许比pfizer还难过，所以现
在象疯了一样到处乱买，买genzyme个人认为是为了未来几年sales不会太难看，
pipeline方面能有多大帮助真的不好说。
感觉后面还会有大动作，尤其是Genzyme买卖搞定以后。
抱歉老是散播悲观消息，不过有事实依据情况下悲观点不是坏事，我在Wyeth工作一同
学，就是及时看到形势不好，提前做了两手准备，最后顺利回到学术界，拿了pfizer

于prevention of stroke in patients with A-fib，or treatment VTE.
antiplatelets such as aspirin 用来防止 platelet rich thrombus formation. 所
以主要用于心脑血管病的防治。机理不一样，所以针对也有不同。
warfarin bleeding risk 远大于aspirin。特别是不能做到有效 routine therapeutic
monitoring 的情况下。

楼主的信息可能我理解错了，我觉得他说的是他家人得过MI,heart attack之类的冠状
动脉clot引起的病，所以问化clot的问题
至于warfarin, aspirin and plavix的比较，warfarin的正确dosing是前提
我的推荐也是有临床试验的study的根据的，上课的时候老师带着我们总结了几个study
,然后跟我们分析为什么warfarin是首选的原因。
warfarin的替代性药源源不断上市，但是新药有新药的问题，而且cost是个大问题
所以现在就说新药替代warfarin还太早了点，在这里也就没推荐给LZ了

在顾不过来。还是要争取把GPA, leadership提高提高比较重要。
再说还有IPPE hours要做呢。
告诉你一个fill的时候也可以学习的小窍门。我们Rx label上一般都有把generic的
brand名写出来，比如: Clopidgrel 下面一行会写generic for Plavix
brand的药瓶上会有用小字写出generic name。
不上班也有好多方法记药名的，可以去quizzlet,flashcard machine之类的网站自己做
falshcard和games. |

楼主，你爸爸这个情况，如果放的是drug eluting stent，aspirin and plavix need
to be on board for at least one year, if they are stopped, there is a higher
risk for acute in-stent thrombosis, comparing to bare metal stent. It can
be life threatening. If it was a bare metal stent, your father needs to
take them for at least a month. Afterward, depending on whether there is
any contraindication, he can continue these two drugs, or only taking ASA. |

different from idiopathic TTP which is generally from an immune process. The

俺们的流程是plavix和coumadin以及Arixtra至少要停4-5天。heparin drip至少停六个
小时。baby aspirin没事。
Solid organ biopsy, such as thyroid, liver, kidney, etc., PLT至少75以上，
para/thoracentesis至少PLT在50以上，不然就得至少输血小板6 units，要不就不干。 |
