threading的问题，结果第一个结果就显示他的blog: Thread.Sleep is a sign of a
poorly designed program。感觉他说的Thread.Sleep坏处就是bad for timing和阻塞
而且无法自由控制。然后此人建议把所有Thread.Sleep换成timer，把原本sequential
的线程工作代码换成timer与event控制机制，有没有高手讨论下这样做到底好处有多大
呢？个人感觉如此一下本来简单的线程会变得很复杂容易出错。 |

Let me keep it real:
I know last year there were 4 Sr.3 in my office, 1 got promoted, 3 were told
either to start looking for alternative opportunities (fired) or to have
one year to improve.
The 1 got promoted is American, the rest are foreigners.
1 of the foreign Sr.3 had a "need improvement" rating on people skills.
Frankly, to be promoted to manager's level, even "meet expectation" is not
enough --- you have to exceed to be promoted.

随便一个化学的PhD，至少毕业都是6万以上，没经验不是理由，
新毕业的谁有经验啊，不过最近的就业趋势是，四大化学慢慢都比较难了
材料化学和高分子还是比较火，我朋友的几个同学，刚刚从普度和麦底讯的
有机拿了PhD，现在都没找到工作。不过定位至少7万以上，倒是高分子一边
在GE，巴思服，罗们好思和东人一直都很吃香

月发现长大了好多，然后做FNA结果 suspicious for a follicular neoplasm (
bethesda class IV)，然后马上约医生手术。
本来是准备直接找sloan cancer center去手术，但是当初大意觉得恶性的可能性不大
，在不确定的情况下去sloan好像有点过虑了，只是找的甲状腺专科医生安排手术。
11月30号手术，12月26号收到医生电话说是癌（期间每隔一周都打电话给医院追问病理
结果），12月27号去医生那里拿到病理报告 “poorly differentiated throid cancer
”,我那个医生行医30多年，说治了上万例甲状腺病人，我这种情况是第一次，当时听
完人就shock了。
回家和我家人商量了一下觉得还要必要找别的专家看看，马上开始和医生们联系，纽约
最好的甲状腺专家们，已经约上了三个，但是其中sloan的专家要等到1月底，另一个

发帖数: 2763 | 27 不好意思，我今天上网看到你的帖子，才想起来你给我发过这个信。回国一段时间，所
以堆在了一起。
因为手术时候没有预计是恶性的，所以可能没有做central neck dissection.
可以再做一次detailed neck ultrasound，当然刚手术以后，可能有局部水肿影响超声
效果。
如果阳性，唯一的办法就是再手术清除颈部淋巴结，poorly differentiated 可能对
radioactive iodine不敏感，Tg不下来，或者淋巴结不消失的话，要考虑conventional
radiation.
很多甲状腺病人都是带瘤生存的，肺转移什么的，治疗办法很有限。现在有几个
clinical trial，可以考虑参加，但是药物副作用比较明显。
我说话比较直，而且我觉得你是很 well educated，所以就有一说一了。实现声明，以
上都是我在fellow时期的经验，开业以后我没有看过类似的病例，看上去你已经有了很
专业的医生。 |

differentiated cancer is very uncommon in young people, you need second
opinion for the pathology diagnosis.
This is so rare that I won't trust this diagnosis only if with convincing
ancillary studies.

出不来，后来他们把我的样本送到New York Presbterian Hospital Weill Cornell
Center再去检查，然后出的这个结果。
不过我的医生也是和我说，理论上讲如果是poorly differentiated cancer 应该是很
严重的情况，但是看我状态以及病情好像也没有这样的厉害，也许我的情况没有那么糟。
这二天一直在和医生联系，拿到我的surgeon opreative note 和 pathology slides，
到时候见新医生的时候给他们参考。 |

moderately differentiated的，不过是diffuse sclerosing variant，所以相对来说
比classic variant更aggressive一点。虽然甲状腺癌相对发展比较慢，但是现代医学
的治疗手段也非常有限，像前面回帖说的如果对i-131不敏感的话基本上是没有办法的
。很不幸我老婆就是这种情况。现在只能定期检查希望肺转移没有长大。clinical
trial的药副作用很大，不到必要时是不会考虑的，而且只能延长几个月的寿命。
甲状腺癌在年轻人中其实发病率不低的，我们认识好几个30岁左右的病人，男女都有。
我在inspire.com上甲状腺癌论坛上也看到几个poorly differentiated的20出头的女病
人。以往甲状腺癌的病例比较少，经验丰富的医生并不多，因此去好的医院非常重要。
其实你去的都是非常好的医院了，应该相信你的医生。这个病最有效的治疗方法是手术
，最好把颈部的可疑淋巴结都清理掉，也方便日后用tg跟踪。若发现肺转移，预后要差

一开始就说“你看，我从一开始就给你不小的剂量，到现在已经是MAX，但是。。。”
当时脑子就懵了，不用说是不好的结果。他给我看，发现只有６个follicles，说还好
有4个比较大，都在13-14mm，应该是不会消失的了，但另外的两个就都比较小。周三或
周四就该打HCG，周五可能就要取卵。医生说我是poor responder。问原因说还是和
endometriosis有关，那些scar tisse and cyst引起的粘连已经影响了卵巢的功能阻碍
了卵泡的发育。然后医生让我决定是proceed还是cancel。想想我已经走到快最后一步
了，打了那么多针，肚皮和手臂都青肿了，换来的竟然着个结果，心里真是不甘呀，为
啥要个宝宝就这么难？我想无论如何我都要继续下去，就算给自己和家人一个交待，不
是说不抛弃不放弃吗？

did lap in 2007. This year I had two failed injectable IUI. The first IUI
cycle I only produced 1 mature follicle but in the 2nd cycle I could produce
