schizophrenia. I've worked in a mental institution before, and he sounds
just like some of my patients. His wife may have folie a deux. He needs to
be treated. Schizophrenia is not curable, but if treated, most people
function much better in society. |

Well, can you tell me how many percent of svd women end up with a vaginal
tear, or a 侧切？ a well prep'd abdominal wound compare to a non prep'd
perineum wound, which one is more likely to get infection?
I have done several postpartum emergency hysterectomy(紧急产后子宫切除）,
all because of un-controllable hemorrhage after SVD. None result from C/S.
a few hundred CC of blood loss from every C/S is a non issue in pregnant
woman.

。 先是出血，出现宫缩， 开始进入labor stage 1 (latent phase)。好疼啊，从来没
有过痛经（考点：prostaglandin, tx NSAIDs）, 对这种痛实在不能忍受。去医院，医
学院附属，有医学生，intern, resident, attending。 告知宫开一指， ok, 回家 （
考点： 宫开 3-4指收入院，上epidural 也是宫开4指； 另外，stage 1, active
phase, cervical dilated>=3cm）。
Chapter 2: 好不容易过了一个晚上，第二天再上医院，该收我了吧，疼得实在不行

发帖数: 40 | 26 发现来这儿的OB 护士很少,这方面的话题也少,就我们医院和我的经验谈一些我的体会:
我们医院OB 分Antepartum, Labor and Delivery, Postpartum三个科, 但前两科在
SAME FLOOR, 我在L&D, 所以有时候会FLOAT 到ANTEPARTUM, 也就是High Risk
pregnancy.我们只在这三科间ＦＬＯＡＴ， 所以不觉的有什么难的． 在国内如果要
你选科室，肯定是金眼科，银外科，又脏又累妇产科，但我感觉在美国做护士，Ｌ＆Ｄ
是相对轻松和干净的，但责任心大，因你是在ＣＡＲＥ 两个病人．护士大多时间是观
察ＭＯＮＩＴＯＲ．和医生的交流也容易的多，大多数时候ＭＤ或ＣＮＭ都在ＦＬＯＯ
Ｒ，尤其是病人的胎心有问题或临近开全的时候，如果你熟悉了常规，很容易的．Ｌ＆
Ｄ护士最多２个病人，有一些时候只有一个．但一点就是，我们ＣＨＡＲＴ特频繁，每
半小时或１５分钟，但也就是有关宫缩和胎心，我们科室ＭＯＮＩＴＯＲ到处都有，（
除了厕所）随时你都可以知道自己病人的情况．
特憋扭ＢＥＤ ＢＡＴＨ的，我竭力推荐ＯＢ :-) |

BORING 不是L&D 的缺点,护士工作12小时,能有这么一段"BORING"时间让你放松一下,不
是很好吗?
不是所有的POSTPARTUM 护士都会向带你的那个护士在床边站整整40分钟的,如果MOM
BREASFEEDING 有问题,我们会叫 LACTATION CONSULTANT 帮她们.
产后护士一点应注意的是,虽然说CARE 3-4 个病人,但实际上是6-8个,包括BABY,有的时
候BABY 比MOM 要难HANDLE,记的一次我FLOAT 到MB FLOOR,一个BABY 吐奶,我人为是正
常现象,但一个老护士告诉我,他如果再这样,就要送NICU (NEONATE ICU).
总的来说,如果你热衷于哪一科或哪一职业,就象你热恋一样,你就看不到他的缺点,你会
全心投入 :-)

Medical center NICU， 还有postpartum 部门很缺人，也要亚洲人。有经验的可以申
请试试看．
我刚有了一个正式的工作，虽然是per diem 的，但是这的人不嫌弃我，还是全心全意
做这个工作吧．也不可能两边跑了。 |

首先“产后抑郁”不等于“产后抑郁症”。
超过50%的新手妈妈在生完孩子之后的一段时间内会表现出抑郁的情绪和症状，但却没
到“抑郁症”的程度，这就是我们所说的“产后抑郁”，产后抑郁只要得到适当和温暖
的支持，可以很快得到缓解。
产后抑郁症（postpartum depression）则是指女性于产褥期出现明显的抑郁症状或典
型的抑郁发作，与产后心绪不宁和产后精神病同属产褥期精神综合征，发病率在 15%～
30%。产后情绪失调也会影响男性，1%至26%的新父亲。典型的产后抑郁症于产后6周内
发生，可在3～6个月自行恢复，但严重的也可持续1～2年，再次妊娠则有20%～30%的复
发率，即以前有过几个孩子而没有患上产后抑郁的母亲，仍在带最小的孩子时患上产后
抑郁。另一方面，尽管怀孕和产后期间可能会发生生物学和心理社会方面的变化，但大
多数妇女仍未被诊断为产后抑郁症。
产后精神病是一种更严重的产后情绪障碍，大约在分娩后每1 000名妇女中就有1至2例

问病史是关键，这个病人没有任何血管危险因素，从不吸烟，没有高血压、糖尿病、冠
心病病史，只有这个肺病，年轻时有一次postpartum DVT.
最近和人讨论，都说要合并CTCA和CTPA，的确见过几个病人两者都做，吃了不少射线和
