其实要多谢楼主写了这么好的帖子，介绍了公司里面很多不同的部门，我才有机会说说
RA的情况。希望我没有客站主位，分散了大家对楼主原贴的注意力。
回答几个小问题，关于oncology和cardiology药物怎么会是一样的这个问题。在
research上面，这个两个当然不同，在development中涉及的相关细节也肯定不一样。
但是，如果从big picture上来看，任何一个药品的研发，都涉及cmc, non-clinical,
clinical 三个部分。在cmc上都要求，逐步建立well controlled manufacture
process 和robust and valid control system. 再non clinical 部门，都要做药效，
药理，药动，毒理试验。毒理试验都需要急性毒理，慢性毒理，致畸性，致基因变异性
等等的研究。再clinical方面，都要做I, II, III三期实验，I期确定药品在人身上的

array/MS似乎还有很长一段路（希望aptamer array能迎头赶上呵呵）。小分子（
glycan
，metabolite，etc。最近出了个sarcosine）呢？lectin特异性太烂。希望aptamer
能顶上？不然也许只能指望HPLC之类能high throughput了？
其次得能把差别和各种physiological/pathological condition联系起来。这个ms进
展也太太太太慢了吧？就说cancer吧，有几个大家都服气的diagnostic/prognostic
/endpoint biomarker？一个PSA用了这么久，还一直被批。
最后要能根据这些差别来调节therapeutic regimen。Y/N型的或差别大的好办，学
herceptin
附赠个kit就是。检测分辨率不够的话呢？最好还是能够下一样的药，对不同的人达
到不同的效果，controlled release？也许就要在delivery method上做文章了？各

BTW, I don't know other ways that may specifically increase the expression
of certain gene variant like RNAa or ASO (except gene therapy, which is
probably more notorious I guess hehe)

Quantiferon我们学校是用于证明你在过去的一年中没有active的感染过，好像目前CDC
不是很推荐，gamma-interferon还是不是很特异吧。。。因为我自己是PPD阳性的，X-
阴性，做observe之前必须要测个quantiferon。。。。 |

PE, PPD +, and I just went to have a CXR, and showed it to health service
it's ng, then I just report each year if I have cough, low fever, weat at
night, and lost weight,etc.
Never taken any INH regimen for chemo.
I think every institute has its own policy. NO universal manditary policy to
take IHN for PPD+ ppl.

down，更多见的是sky high.所以如果原来已经用basal/bolus regimen的病人，术前把
basal insulin 停掉，肯定是要引起高血糖的。
按风专家和yf Chief的指导原则，我们具体问题具体分析。按我个人的有限经验，术前
头天晚上停一次basal insulin而引起第二天凌晨高血糖的,主要是自身basal insulin
分泌不足的患者，譬如1型糖尿病。可事实上，有很多用胰岛素的2型糖尿病患者，主要
问题是餐后胰岛素不足，而basal insulin分泌还可以。这类患者的表现是餐后高血糖
，而空腹血糖不很高。一个间接证据, 是当初在国内，没有 Lantus的时候，很多患者
只给三餐前bolus 短效胰岛素，空腹血糖照样控制得很好，靠的就是残留的自身basal
insulin分泌。那时候，这类病人若要上手术的话，头天晚上也没有Lantus，第二天凌
晨也没见空腹血糖高是个问题。这类病人说实在的，平时就不需要给Lantus，可事实上
，这边现在不知咋搞的，只要一上胰岛素治疗，个个病人都从Lantus开始。你说的这个：

ICU的病人血糖高，如果以前hgba1c正常的，一般是stress induced hyperglycemia。
人在术后，以及critical care的时候激素，和正常的时候是判若两人的，非常insulin
resistant，这时候要控制血糖，只有用insulin.从最初的宁可高血糖，到后来的80-
140，到现在把goal定在140-180，但是控制血糖从来都是应该努力做到的，特别是对胸
外科手术以后的病人。
insulin sliding scale，是内分泌医生最讨厌的regimen,很难有真正效果，也不符合
科学原理，所以会有楼主所说的前后似乎没有明显效果。但是因为容易使用，非常普遍。
还在ICU的病人，如果血糖持续高于180,一开始应该上drip，然后血糖稳定以后，
transit到subQ insulin.
如果tube feeding, continuous feeding,给regular insulin Q6hr scheduled,如果

Pharmacists in China only dispense drug, and they don't involve in the
patient treatment process at all. In US pharmacists actually work with
physicians as a team and recommend drug regimens to the physicians. The
curriculum in China seems to train Scientists rather than Pharmacists. They
(including me) learn very minimal knowledge about disease state management.

efficacy.
Go to GNC you will get much better products.
And get a few fish oil with CoQ10, that's better dietary regimen for
cardiovascular health than Nitric oxide related product.

定量不是定性。定量是为了算PK参数，然后设计合适的dosing regimen，这样你才有可
能在临床实验中看到efficacy和尽可能小的side effects。Biologics的off target
toxicity比小分子小多了，但也有immunogenicity等问题。
Discovery不太了解就不评价了，biotech也不太了解。 |

，那很有可能就不行了。
不是什么病都有特效药的，特别是到了现在这个地步。
而且就算能买到药，你知道吃什么strength,dose,frequency么？这种事情不是你写在
网上我们就能做出therapeutic regimen recommendation的。
你还是告诉医院让他们治疗吧。如果不行了也没办法。而且会死的很突然。
