there is no cure for rheumatoid arthritis, but start therapy early could
help induce remission, can conside DMARDs or NSAIDS and maybe local or low
dose systemic steroid, and get started on physical and occupational therapy
early, |

would be infectious, non-infectious inflammatory/connective tissue disease/
vasculitis, or cancer,
history would be important,which include travelling history, animal exposure
, medications and such,
in addition to routine chest x-ray, urinalysis, blood cultures, cbc and
liver function test, you probably want to look into sed rate, crp, hiv test,
hepatitis panel, tb test, ana, rheumatoid factor, SPEP and/or UPEP, CT |

mostly used for detecting cancer activities, i think there is an
experimental study evaluating PET scan in detection or follow up of
rheumatoid arthritis,
PET is very expensive and has large amount of radiaiton exposure,
I would think he needs a MRI of the brain, and if the MRI of the neck was ok
(that is, no spinal cord compromise), then he might need EMG or a spinal tap

then are you sure you had strep throat? was strep screen positive or throat
culture positive? you might had mono instead, did you get a mono screen? did
your doctor check your liver function test?
if it's not mono either, you'd need a TEE to rule out endocarditis and get
some basic labs for autoimmune diseases such as ANA screen and rheumatoid
factor, it's basically a fever of unknown origin work up, |

well practiced here in the United States. There are many effective
treatments that may not cure the disease but can significantly relieve
symptoms and slow down progression of the disease.
I don't think you necessarily need to go to big medical centers. But you do
need a referral to a rheumatologist. |

available in China, including some of the newer DMARDs as MontBlanc
mentioned. Make sure you get to see a rheumatologist (风湿科医生)。 But
that is the situation I heard in big hospital in big cities, I am not sure
about smaller cities.
Rheumatoid arthritis is a disease that many effective treatments are
available. Delayed treatment will result in irreversible damage to the
joints. Don't sit on it and wait :) |

孩子9个月大时就出现了关节肿胀，由于没意识到问题严重，11个月时才开始入院，现
在一年多了。
目前的症状是多个关节出现肿大，不发病时可以走路，但情况非常依赖药物（Naproxen
），不吃药的话睡觉都会蜷成一团，十分可怜；总吃药的话，孩子很容易感冒发烧，有
时高烧不退，非常危险。
早些时候用过抗风湿药副以少量激素， 效果差，副作用大，才改用生物单抗类药的。
想问问这种病可以长期用药吗？有没有比较好的治疗手段？国外尤其在美国市场有没有
好点的药，能不能直接买到？

目前治疗JRA也只有这些办法了。生物制剂也不能一直长期用下去，定期复查定期随诊
。多活动锻炼，尽量减轻并发症。 |

家族史
母亲：2008年元月因突然双臂疼痛，由医院诊断为类风湿性关节炎，当时用激素治疗了
一个月，其它的药吃到当年的8月份停的，到当年的10月份基本治愈；之后一直自我感
觉良好，未在检查、治疗。直至2013年初，因发热去医院检查发现肺部感染并间质性改
变，白细胞、血小板减少，作进一步检查最后确诊为红斑狼疮，现服用美卓乐及羟氯喹。
弟弟：十六岁左右确诊罹患牛皮癣。
侄子：今年21岁，日前确诊罹患牛皮癣。
病史
1995年的4、5月份患带状疱疹（Herpes zoster）
1996年初，开始出现乏力、精神差等症状， 1996年6月确诊为慢性淋巴性甲状腺炎合并
甲状腺功能低下，同时开始做甲状腺素替代治疗。
1996年8月初开始出现不明原因的发热，此后常有连续几天的低热，用抗生素和抗病毒
药物治疗无效，但查不出原因。

tapered and then stopped while other medication (disease modifying drugs)
starts to work. Or 激素 stays at a low dose as possible. but she need to be
on other medicines, the most commonly used here in US is methotrexate. If
not working, there are other medicines too. you can ask her whether she has
tried methotrexate before. |
