a oncologist, I am here to give my two cents though I haven't seen the
patient yet.
First: What is the type of lung cancer and what stage? This is very
important. If patient has stage four squamous cell carcinoma. There are
limited chemotherapy options. The chemo agent that can be used are Cisplatin
or carboplatin in combination with paclitaxol or Gemcitabine. Some old

but the incidence in squamous histology is quite high on phase 2 study, it
happened in 4 of 13 patient which is kind of make sense because squamous
carcinoma is more centrally located in contrast to adenocarcinoma, therefore
the bleeding side effect will be more pronounced and more fatal. |

adenocarcinoma, 20% of squamous carcinoma, 15% of large cell carcinoma and

Which is the same as CIN1
Change s are seen in the cells that may show sign of precancer. LSIL-The
changes are mild.
For many cases, the CIN1 could go away by itself if it is not severe.
However, you need follow up for like 6 monthly frequency pap smear. Dr. may
also suggest for further valuation or treatment.

why we need to carefully stage each patient and offer the treatment
appropriate to that stage. Both over and under treatments are bad.
BTW, your doctor probably mentioned already, you should stay away from
smoking, alcohol abuse, and hot tea/water (something culturally unique to
Chinese). |

show abnormal results for the first time:
"High grade squamous intraepithelial lesion (HGSIL) encompassing moderate
dysplasia, severe dysplasia and carcinoma in-situ"
According to the doctor, it's a sign of severe pre-cancer condition and she
asked me to schedule a colposcopically directed biopsy ASAP. It is also
very likely that a LEEP procedure need to follow after the biopsy.

有红细胞，除了pH值高了一些。没有任何症状。但cytopathologist的报告有点迷惑 (
报告如下），医生要我去看'urologist':
URINE, VOIDED: NUMEROUS CLUSTERS OF UROTHELIAL CELLS, SQUAMOUS CELLS
ANDDEBRIS; A LOW GRADE UROTHELIAL NEOPLASM CANNOT BE EXCLUDED.
这是一种很不正常的状况吗？cancer 的可能性有多大？有可能还是可能性很小？

treatment patient got so far is very similar to what he would be treated in
US nowdays. I can tell you that. If Abraxane fails, I would suggest to
challenge him again with Gemcitabine or perhaps Nevelbine. You could try
Tarceva at some point but do not expect a dramatic effect, the response rate
is probably less than 5%. |

发帖数: 63 | 32 家人，男，长期抽烟，62岁。2015年9月CT检查右上肺癌伴右肺门、纵隔、右锁骨区淋
巴结转移。肺穿刺病理示：右肺上叶穿刺组织示恶性肿瘤，结合免疫组化结果提示鳞状
细胞癌。
行GP方案化疗两周期。 CT检查，肺部病灶无缩小，纵隔淋巴结增大，后转上海肿瘤医
院遂予肺部病灶放疗+同步化疗6次(多地他赛40m&+顺铂40m&)。患者2016年3月CT示右肺
上叶结节较前缩小，阻塞性炎症较前略明显，两肺新见多发小结节，左侧锁骨上区小淋
巴结。2016年4.1一4.15，两次予以PD一1单抗300m&静滴(0PlDlV0)方案治疗。
我知道放疗和化疗基本是对的。两个问题：
（1）Squamous Lung cancer 现在有没有特效药物，或者靶向药物？
（2）免疫治疗有没有用？
患者准备前往中山医院去做肿瘤基因组测序。不知道有没有用？

我父亲陪我妈来看黑色素瘤，可能自己也长期劳累过度；上上周二出现无痛全程血尿，
上上周三我们看了社区医院urgent care,np只做了尿检。安排上周三b超和周五见医生
。医生觉得b超和尿检不说明肾上有肿瘤，可是也无意解释全程血尿一般说明是肾上问
题。 urgent care医生约了泌尿科医生，但遥遥无期。也不允许我们做ct 检查。请求
专业人士给予指点。
1）b超中说cyst是囊肿还是肿瘤？“3.6cm cyst is noted in the lower pole. 9mm
nonobstructing calcification is noted in the mid to lower pole.” 3.6cm和钙
化是不是说明时肿瘤？
2）如果有肾肿瘤，b超是否可以发现？还是必须做增强ct？
3）如果是肾癌，这种情况是不是应该立刻回国手术？
家里悲剧一个接着一个，有点举足无措了。谢谢大家。。。

患者44岁，男，6年前因为肝癌做了肝移植 （符合米兰标准），之后无灾无病好几年。
去年2016年初发现轻微贫血，血色素12，因为当时在服小剂量阿司匹林，于是停服，看
看有没好转。托托拉拉到了年底，还是贫血，血色素10.5。医生说做个内镜。于是在胃
内发现一个肿瘤，内镜医生直觉是GIST。随后CT，但CT却没有异常。4月初第二次胃镜
，GIST依然还在，取样活检表明可能肝癌转移或者是非常罕见的HAC。第二次CT 报告大
小46x35mm，说比较第一次32mm变大了。可能第一次看片医生那天喝多了？肿瘤位置在
Fundus，非常接近GOJ。
AFP好久没有查了，因为肝上的情况一直很好，肝移植前AFP 100多，移植后AFP一直小
于2。上一次至少是2，3年前。也是疏忽了。现在AFP 254. 显然胃上这个要么是转移，
要么是HAC。
下周二医生还要安排一次PETCT，周三见外科医生，估计排除转移外就安排切除了。 求
这里的专家教授指点，这个情况如何是好？下面我贴上最近几个报告。

I got a abnormal pap smear result last week and freaked out. Mine is said as
"rare atypical squamous cells of uncertain significance", also called "ASC-
US".they didn't do the HPV test since the sample was not enough. My OB asked
me to come back in 4 or 6 months to re-do the pap smear. When I asked her "

show abnormal results for the first time:
"High grade squamous intraepithelial lesion (HGSIL) encompassing moderate
dysplasia, severe dysplasia and carcinoma in-situ"
According to the doctor, it's a sign of severe pre-cancer condition and she
asked me to schedule a colposcopically directed biopsy ASAP. It is also
very likely that a LEEP procedure need to follow after the biopsy.

all kinds of skin cancers, including melanomas, squamous cell carcinomas and
basil cell carcinomas dramatically. Don't recommend to do it. |
