标 题: 麻痛症状多年,求帮助

我的左侧身体一直都有多处不适麻痛症状，看了多年的各种医生，也查不出问题.断断续续做了不少物理治疗，也不见根本好转，很象是肌肉劳损或pinchednerve的症状,估计和这些年在电脑前紧张忙碌上网太多有关,但这如何才能确诊呢?
具体来说说我的历史:
最初的时候大概6,7年前了,有一天突然觉得左脚底前外侧麻痛,象蚂蚁在那里面
爬,于是我开始了我的求医问药路程,该做的检查都做了,象X-Ray /CT /MRI /EMG,
查血,查尿等等.后来就开始做了一些物理治疗,断断续续的,也没啥根本好转.有那末一短时间,好像左侧lower back的肌肉也常麻痛.
后来这几年症状有些变化,麻痛感主要沿膝盖外侧,延左小腿外侧,一直到脚踝外侧上面一点的肌肉,然后穿到脚底板,在脚底板中线靠左一点的一条线上麻痛,burning,tingling感觉,有时脚底那个叫地筋的 |

络不通会导致半边不遂。
问题很多，试图说清楚。女，年龄mid 30s，不怎么运动，但正在改变。
最急迫的问题：
大约两年前开始的，半夜睡着时左手心出汗，右边很干燥。大半年前开始恶化，除了手
心出汗外手臂有些numb的感觉-都是半夜睡着后，不存在压手的问题。最近开始手脚开
始有tingling的感觉，白天偶尔也会有。耳鸣了大半年，查了脑部MRI，说没问题。吃
了点OTC的调节耳朵神经的supplement，有好转。左腿关节痛，左手手指关节有时会痛
并发热。
--那种半夜醒来半边手麻脚麻
前段时间自己有时会做做艾灸，因为不懂，就灸腰部，阴陵泉，左手为主。最近开始看
中医针灸，说阴虚，有些气虚。我就停止艾灸了。可是昨天第二次针灸完后，情况变坏
。麻得更厉害了，平时白天不麻的，今天脚到现在都难受，象蚂蚁咬的感觉但没那么厉
害。
其他问题:
肾不好，腰酸背痛，有时腰酸得厉害。Hold不住尿，非常尿频。
胃不好，极容易饿，可吃多了又会胃胀打嗝。国内诊断是胃炎，美国诊断是reflux
咽喉炎。

那篇文献里提到：H+ Ca2+竞争transmembrane calcium transport system同一结合位
点。所以H+减少，and/or Ca2+增加会导致血管收缩。
但是BRS physiology 4th edition Page 185提到：H+ 和Ca2+ 竞争albumin结合位点.
所以呼碱时，H+减少，更多的free calcium将会与albumin结合，血浆free Calcium浓
度降低，导致tingling, numbness和muscle spasm.
是不是说free Calcium对骨骼肌和平滑肌的作用是不同的？ |

其实，我真正复习的时间就一周。 主要就做了KAPLAN的七套题。
谈谈我的一些考试体会，总结为七种武器。
第一招：吸星大法。
认真做七套题的每一道题目。不要在乎你的了多少分。我做的最差的 50%好都
不到．正式考也是75道过。 重要的是看你做错题目的RATIONAL。 NCLEX的考点就那么
多，基本上七套题都覆盖了。吸取了七套题的精华，您就能过。
第二招：无招胜有招。
真正考试的时候，用到ABC，和马斯洛的只有1-2道题。技巧是当你是在走投无
路的时候才用得， 如果你能用自己的知识判断作出选择，就不要依赖技巧，因为技巧
性的东西都缺乏灵活性。生搬硬套有时候反而容易走入魔。
第三招： 谁最先死
做“最先看谁”的题目，就这样想，如果你谁都不去看，谁可能最先死。注意
一些敏感的词，在NCLEX里，这些词往往比较严重 tingling（一般有缺血，或者电解质
紊乱如Ca) lethargy (一般缺氧，或者低钾）。 disorientation(合并CNS，病入膏盲）

上次开始吃Venlanfaxine是在日本地震后，反正就是每天看电视看评论不能睡觉有些焦
虑，吃了这个药后就好了，这个药的副作用：
1。做梦，梦像真的一样，还大声说梦话。第一个梦就是跟好朋友的前老公吵架，醒来
想想人都离婚又结婚了，我这操哪门子心哦。
2。肌肉会有tingle的感觉（很轻微的）
3。关节疼，早上起来下楼梯困难。这个是我停药的原因。
这个药的好处还有一条：消水肿。
停药的过程还算好，也许是我准备充分，先隔一天吃一个，再隔两天。。。这样慢慢减
，虽然也有头疼失眠但在可承受范围内。
总的感觉这药挺有用的。 |
