发帖数: 431 | 3 我刚破处的时候是不早泄的，在国内，经常大战30分钟，还可以一晚上连续作战多次。
。。
后来来美国呆了几年，可能是锻炼不足，课业压力也大，时间越来越短，连续作战能力
也没了。。以至于去年因为早泄想自杀，活塞运动3分钟就不行了。
后来我因为不相干的原因看了心理医生，开始吃antidepressant，然后上网查资料，意
外地发现SSRI可以治早泄。但我一开始吃的lexapro没有效果。
我就又去找了urologist，他建议我试试paxil，也就是国内说的帕罗西汀。
一开始一个礼拜仍然没有效果，我以为没戏了。结果两个礼拜后，神奇了，我不仅恢复
了以往的时间，心理整个焦虑、抑郁的毛病也都没有了，床上很自如。
如果谁有类似的问题，也可以试试这个paxil。 |

今年3月份的时候，
洗澡时无意发现右侧阴囊内有肿块，不大，也没什么感觉
还是约了urologist检查
医生安排了ultrasound，说有一个大约6mm的肿块，不在睾丸上
医生认为是附睾炎， epididymitis
什么药也没开，说没事
也就没太在意
可是慢慢的，几个月过去了，
开始渐渐的有疼痛感，并扩展到了下腹，
现在左边睾丸内也有了一个肿块，还有就是感觉有些腺体有些肿大
和医生说了，下腹也疼
医生就安排了CT，检查肾脏
也没查出个所以然，就是说左边肾脏有3个小小的cyst，但是也问题不大。右边肾脏无
异常。
可是我是右边疼的厉害
中间有很多次尿检，没有一次检查出有异常。至少一次医生也没有提到过。
现在就是，两边阴囊内有肿块，每天都有疼痛感。有时候阴茎也有疼痛感。尿道口有红
肿。下腹以及
后背有疼痛感。

Department of Urology 好象内科外科都包括了。
主要是这几天带家人去看Urologist, 最初来的是个黑人瘦高女孩住院医给检查的。挺
佩服的人家的。

囊炎如不及时手术引流,进展非常快,很容易导致感染性休克,最后导致多器官衰竭.保守
治疗无非就是补液,纠正电解质紊乱,抗感染抗休克,解痉止疼.奶奶已经有两大器官衰竭
,预后是不是乐观,真的不好说.看你们怎么想了,如果真的不愿放弃,可以继续要求透析,
止疼等辅助治疗,至于能挺两天还是七天,谁也不会预测那么准.BLESS 你奶奶,希望奇迹
出现.说的比较直,望见谅. |

请朋友帮我找个比较nice的MD，有空去做volunteer这样他/她可以帮我写推荐书.但是
介绍的这个医生是Urologist 和我所在的医院有contract,所以我不敢轻易联系他，一怕
医院误认为我才有个工作就想跑（没办法，只有一年工卡,还要靠这个工作赚学费呢）
，二怕要是volunteer做不好反而得罪这个医生。
我自己也开始认识一些医生，有几个特别nice.我不知可不可以找个帮我写推荐，或
者也会有以上的问题。
还有啊，找哪个specialty 的 MD 为我NP program写推荐比较好。 |

不过医学上的有些决定也没有绝对的对错，医生也只能在权衡利弊下做出当时看来最好
的选择。如果我们从医生角度来看，手术直接切除，没有伤到输尿管没什么，但如果伤
到了那可能真的就是医疗事故了，病人可能会责问当时为什么没有找urologist，本来
这个是可以避免的呀。
换医生的话，如果确切地知道哪个妇科医生有好的口碑的话也可以考虑。不然的话感觉
意义就不大了。 |

我老板消化不良，老是拉肚子，做了3次肠镜，3次胃镜，1次PET-CT。最后被告知是过
敏，不要吃dairy的东西。
肾脏上一个cyst，我出国前就有了，来这里体检，让我做个CT看看，我比较闲，心想要
狠狠占保险公司便宜，做了，增强了10个啥值。（后来自己查查，说是这样的增强可以
忽略的。）让我做MRI，我又很闲，心想找个牛人看看，约了个某牛校urologist的
director，看了，增强MRI做了，说没啥，让我半年随访一次MRI。我不打算去了，MRI
太吵了，打造影剂的房间太冷，护士也太丑。
下个目标是要找个机会做次PET，人生才完整，哈哈~~~ |

aggressive biopsies.
Old recommendation is PSA screen yearly for patients >50 years old (that's
ACS guildline) with or without DRE. As PSA is not sensitive or specific for
prostate cancer, which create a huge amount of negative biopsies.
There are specilized urologist groups devote to prostate biopsy and hire a
GU pathologist. That is a very lucrative business and draw a lot of
criticism from academic centers.

有做urine Culture: Greater than 100,000 cfu/ml Escherichia coli
urgent care的医生和小孩自己的pediatrician都提到VCUG，跟你说的一样：第一次UTI
并且ultrasound检查没问题就不用做VCUG。
一个困惑的问题是：urgent care的医生好像是说等10天药吃完后再去做ultrasound；
为什么儿医地上天就叫做？
凭目前的检查到底能不能判断肾脏有没有受到感染？有没有后遗症？
还有，怎么预防复发？平时小孩照看得挺好的，小孩每天的奶喝得挺多的(~24oz).

, renal calcs, left renal mass and bladder mass. Good tech may see the left
adrenal lesion but most won't. Then you will still order a CT or MR for
further delineation. Ultrasound doesn't have radiation, but not as accurate
. However, you can see the ureteral jets to the bladder on real time to
evaluate obstruction, which u can't see with CTs

部钝痛，躺下有缓解。面色差，最近体重无下降（本来就很瘦）。
刚约了urologist和nephrigist,要等几天。家庭医生觉得cancer可能性不大，年龄还小
。但症状蛮像。血压正常，无身体浮肿，怕冷（一直怕冷）
是急性肾炎嘛？还是膀胱问题？又经验得给点hint。

去年10月份第一次开始看不孕专家。运气不好，碰见一个不靠谱的医生。她让我lg去做
精子检查也给我做了血检，但是没有做ultrasound。当时太天真了，一味依赖医生，虽
然自己嘀咕应该做个b超的，她没做我们也没抗议。结果发现lg精子太少，活力不够，
畸形率太高，以为主要问题在他，虽然医生提到我的fsh在临界值（10），当时也不懂
这是什么意思，她没解释我也就没问，一门心思的就光顾着注意lg的问题了。当时医生
说，一方面lg应该去看urologist，一方面我们可以做clomid＋iui。这样我们从12月开
始做iui，同时lg今年一月份动了手术。中间因为要出差什么的有两个月没有做。lg的
精子一开始数字真的很差，不到10，但到三月份数字好看了，到了35。但那次（第四次
）iui还是失败了。
然后因为lg的情况好转，我们很觉得前途光明，而且因为老出差没时间，就想自己试试
natural，没准现在也能怀上了。但是不幸的是，我都一直没有动静。这时我对我的医生已经开始不满，因为这么长时间我们都根本见不到她。终于今年6月份我们约了她再次

以前ob让lg在其clinic做sperm analysis， 说是low normal可以怀孕， 结果我7个月
没怀上， 在这之前已经试了10个月。 这个ob真是害死人， 当然也怪我们太相信美国
医生了。 以后凡是都问问2nd opinion很重要， 希望看到此贴的jm千万要吸取我们的
教训， 不要浪费宝贵时间啊。
在我妈的建议下， 逼着lg去看urologist， 做全面sperm analysis， 结果打击巨大，
合格精子头only13%根本怀不上， 诊断是精索静脉曲张， 2个建议做IUI/IVF或做静脉
手术。
想问问有没有类似经验的jm的lg是做手术呢， 还是就直接上IUI/IVF了？
我的想法是先做手术以提高合格精子头到正常的30%， 然后试自然孕， 怀不上再做IUI
/IVF。 做手术后恢复可能会耽误点时间， 可我觉得值得， 不知对不对？
lg今年40了， 我下个月34.
