还有她平时吃什么药？吃Aspirin太多也会引起耳鸣。

Tinnitus(耳鸣):
Continuous or intermittent alien noise in the ear:
-Subjective
-Audible only to the patient
-Steam escaping from a small pipe, ringing, roaring or humming
Causes:
-clockage of the auditory canal
-exposure ot high noise levels
-Acoustic trauma
-systemic disease
-drug toxicity
Refer all complaints of tinnitus.

Her vitals: afebril, HR 56, BP 105/68, RR 12, Sating 94% on 2 L NC.
Her UA showed >20 WBC, positive nitrites, positive LE and culture eventually
grew out E coli. LFT showed albumin 2.5, TP 4.6, otherwise unremarkable.
She had a million of things going on, however, I believed "nausea" is the
key. She didn't have any vertigo or dizziness or vision change, which made

condition where the contralateral vertebral artery is stenosed, compressed,

portion of the cranium, then patients are going to have issues with sight,

controversial.
The key is what caused syncope?
Left carotid artery stenosis usually is not the cause of syncope. Basilar
artery stenosis, however, could induce brain stem ischemia and cause vertigo
and syncope. I did not see basilar artery flow direction on the U/S. If retrograde, a
subclavian steal could cause the syncope.
So you may fix the carotid artery stenosis but not the syncope. The patient

complained of a sudden onset of vertigo then LOC for 1 min. She was very
confused at the admission.
PMH: HTN.
SH: No smoking/ETOH
FMH: HTN
ROS: not remarkable except mild confusion.
PE: VSS stable. BP: 140/80 bilaterally per record.
OA x 2 and CVS:irregularly irregular. Other exams were not remarkable.
CNC/CMP/PT/PTT: nl
Cardiac enzyme: neg
EKG showed A-Fib （new onset).
A CT of brain was neg.

前两条和后两条就是成为美国医生必须的，有没有这个能力另当别论，但别跟我说你上
医学院考board是为了公益。
3.瞎掰。 作为solo，你会看你银行的钱是不是多起来，这个上网的时间恐怕就是你说
的70%，钱少了，让manager处理就是了。
8.外行，spouse管钱就好了，manage clinic你怎么着也得请老美，而且是对你忠心耿
耿的老美，让老美对你忠心是你的本领，而让spouse管clinic是一大忌讳。
10. Zestoretic是brand， 所以有sample， 而且copay是0， 你说你是用generic花
copay呢还是先用brand for free， 如果没有副作用，再refill with 0 Copay呢？当
然你的病人如果都是medicare/medicaid就算我没说。

有点象get low那种电影，“小”却有深意。因为它优美传神的中文译名，自电影诞生
的当年我就知道它，那个时候jodie foster 多红啊。。。
故事尚算能自圆其说，但剧本仍嫌不够丰满，Richard Gere 依旧是美国绅士的感觉，
Jodie Foster冷峻疏远的气质在偶然的爆发中更显热烈真挚。音乐象那句I know
because I never loved him the way I love you 一样感人，出自好莱坞配乐大师
Danny Elfman 之手，以前好像因为喜欢一部电影（Good Will Hunting？）的配乐查过
他的资料，发现他给好多比较特别的电影配过音乐。
再说一句Jodie Foster，她是我欣赏的人，对自己的职业和人生规划如此成功，并且具
有独立完整的人格。Contact 之后淡出，孩子大了渐渐回归，Polanski 的Carnage，肥
温都替她跨刀，期待。

auteur理论最开始媞出来的时候，好像只是强调导演镜头有没有自己的风格。但你这个
看法我觉得很有意思，的确是可以分出来只导和编剧导演一体的，你给分析一下，这两
种导演真的风格不同吗？
你的例子里面，只导的：李安（李安早期的推手饮食男女都是他自己的本子），库布里
克，希区柯克（这个我加的，我记得他主要是导的，查了vertigo和psycho，都不是他
自己写的本子）；好像侯孝贤拍片也主要是用朱天文的本子，两人还有绯闻。
编剧导演一体：伯格曼，杨德昌，昆丁

大概十几部,不过只有后窗，爱德华大夫和知道得太多的人是在美国买的，在国内还是狠
便宜的．在这里见过vertigo,27刀，没舍得．也不算太多，拿来作挡箭牌不甚有说服力，
惭愧．:)
