发帖数: 246 | 3 Gabapentine is not really usefully for the acute neuritis phase. Instead it
will help if it's post-herpatic neuralgia.
We usually should always first try with Tyleno and NSAIDs, then Percocet
kind of medication. Like Nabla said, we need to be cautious to use
Amitriptyline, especially with old patients. If any other medication I
mentions previously does not work, we may think of Amitriptyline. Most of
Dermatologist don't use Amitriptyline for zoster. |

trigger. More often in elderly patient (> 80?) or immunosuppressed patients
, maybe due to weak cellular immunity.

（CBC,RUBELLA AB IGG, VARICELLA ZOSTER VIRUS AB IGG, COMPREHENSIVE HEPATITIS
PANEL, TEST FOR LATENT TB, ANTI-HIV)，说查出来缺什么vaccine就补。
没想到问题来了，医生给保险公司发账单时，用的diagnosis code是“V26.49”代表“
pre-conception counselling".（此种诊断应该全报） 可是我的保险公司Anthem却说
这个code输进他们的系统里是”infertility“不孕的意思，只能负担50%，这下上千块
的账单就砸到我身上了。
急切的请教各位大侠，这个code到底用得对不对？怎么医院和保险公司会对同一个code
有不同的理解呢？
还有姐妹们去做孕前咨询时，医生也让做免疫检查了吗？保险公司给报了吗？我该怎么

，只有一个小红区域和一个快掉的疤。当天去了麦当劳play ground的tunnel里滚爬玩
耍 （又一大错误！有伤口不要去公共play ground的tunnel爬！！）。
２１号（星期二）突然恶化，出现红肿和水泡。周三（２２），周四（２３），周五（

病人女，49岁。
家族史
母亲：2008年元月因突然双臂疼痛，由医院诊断为类风湿性关节炎，当时用激素治疗了
一个月，其它的药吃到当年的8月份停的，到当年的10月份基本治愈；之后一直自我感
觉良好，未在检查、治疗。直至2013年初，因发热去医院检查发现肺部感染并间质性改
变，白细胞、血小板减少，作进一步检查最后确诊为红斑狼疮，现服用美卓乐及羟氯喹。
弟弟：十六岁左右确诊罹患牛皮癣。
侄子：今年21岁，日前确诊罹患牛皮癣。
病史
1995年的4、5月份患带状疱疹（Herpes zoster）
1996年初，开始出现乏力、精神差等症状， 1996年6月确诊为慢性淋巴性甲状腺炎合并
甲状腺功能低下，同时开始做甲状腺素替代治疗。
1996年8月初开始出现不明原因的发热，此后常有连续几天的低热，用抗生素和抗病毒
药物治疗无效，但查不出原因。

孕妇由于情况特殊，因此使用疫苗也应该更加谨慎小心，需要仔细权衡利弊。简单写个帖子跟大家探讨一下孕妇的疫苗问题。
CDC真正推荐孕妇使用的疫苗其实只有一个，就是流感疫苗。此外，CDC常规建议孕妇考虑的疫苗有两个，乙肝和Td（Tetanus和Diphtheria）。在此基础上，出国旅行的孕妇建议考虑加两个疫苗脑膜炎和狂犬病。

由于疫苗的种类我已经介绍过了，所以不再详细介绍。一般来说，除活疫苗之外，孕妇都可以安全使用。活疫苗由于至少在理论上有侵犯胎儿的可能，因此不建议在孕前一个月和孕期使用。美国的活疫苗包括：流感喷剂（注射剂是死病毒疫苗）、麻疹、风疹、腮腺炎（不是MMR，是单独的三种疫苗制剂）、BCG、牛痘病毒（Vaccinia）、黄热病（Yellow Fever）、水痘（Zoster）。这些疫苗使用的时候要小心。但是万一真的在孕前一个月或者孕期使用了这些疫苗也不用害怕，临床上并没有因为使用疫苗而引起的胎儿异常。

小妹要准备去做Medical Exam，填I-693表。我已经做了血检，TB检查，对chicken pox
有免疫，没有syphilis 或HIV。可是对有些疫苗还是不懂，不知道是不是必须要做的
。怕到civil surgeon那里有说这些那些需要做没有做的。所以想把我的问题发到这里

Vacination的第一行：DT, DTP, DTap (这个是什么，要做吗）
第二行：Td, Tdap （这个做过了，打的是Tdap）
第三行：OPV, IPV （这个是什么呢？）

Pneumococcal （好像成年人不用做，是吗？）

是不是要补上Mumps和Checken pot就行了？
另外当时做一个叫啥Quantiferon TB Gold 的是Negative，但是没有做TB，不知道行
不行。
所以体检就是三针疫苗（假如都要打的话）+一个TB test就完了是吧？ |

先付125完成TB皮试，Syphilis（验血），和精神状况评估。这个信息和693表是一致的。
预防针要求的是MMR,Varicella,和Influenza（因为现在流感季节10-3月，有网友已经
终结过）。医生说如果证明10年内打过MMR和Varicella，可以不用再打。Influenza需
要是最近打的。不是所有医院认可你自己提供的证明。
国内来得小黄本一般有MMR的记录。中国人应该都种过水痘吧？怎么证明呢？在美国生
了孩子的女同胞，你可能可以找到证据。你一般会作过Varicella-Zoster Virus test
和.这个test的结果会告诉你是否有抗体。
正如大家说的，年龄和地区不同也许会有差异。还是多问几家医院吧。 |
