2. Birth Certificate for Child with English translation.
3. Marriage Certificate of Applicant and Spouse.
4. Translations of any documents not in English, with sworn affidavits by
translator.
5. Copies of Passports for each family member immigrating – All Stamped
Pages.
6. Copy of current Beneficiary’s I-94 Departure Records (Entry Card), front
and back for each family member immigrating.
7. Copies of all nonimmigrant Approval Notices to document maintenance of
status for all time spent in the US
8. Immigration Passport Photographs (CVS or Walgreens):
6 Photos - Applicant
6 Photos - Spouse
4 photos - Child
9. Medical Examination for each family member immigrating, regardless of age
, by one of the physicians on the CIS list of approved physicians.
Examination Reports, including supplemental form regarding immunizations,
must be submitted in envelopes sealed by physician. Be sure corresponding
name is written on outside of each envelope. Attorney will provide
physician list. DO NOT OPEN THE REPORT ENVELOPE |
