4. A copy of H4 passport and H4 visa
5. A copy of H4 I-94 Form issued at port of entry ( get it online )
6. A copy of H4 approval ( I-797 ) including I-94 in it
7. A copy of spouse H1B petition ( I-797 );
8. A copy of spouse H1B I-94;
9. A copy of spouse H1B passport (all bio pages);
10. Marriage certificate;
11. Last 2 paystubs; and
12. H1B holder Employment Verification Letter from Employer
