1. Class of Admission
2. Date of Admission（This date relates to either the date you were

date your status was adjusted to lawful permanent resident by USCIS or the
immigration court. ）绿卡已经丢失，没有复印件，请问哪里可以找到这个日期？
3. Where was your immigrant visa issued or which USCIS office granted you
adjustment of status? 这个是填Texas Service Center么？
4. Did you enter the United States with an immigrant visa?
这个是填Yes还是No？身份是由F1转为H1B
5.Since you were granted permanent residence, have you ever filed Form I-407
, Abandonment by Alien of Status as Lawful Permanent Resident, or otherwise
been determined to have abandoned your status?
这个应该是No吧？
6.另外Bio Information，身高，眼睛颜色等等需要跟哪个表match么？
