I caught the mistakes of many people before (from clients' side or our own
side) and nobody has ever apologized for anything. They just correct the

boss is, what kind of mistake, etc. Anyway, I'd say to be frank and
straight is usually a good strategy. The point is you want to let the
other party know that measures have been taken so similar mistakes
would not occur again. If I were your client, I'd be much more
concerned with the concreteness of your measures than the way apology
is expressed.
