-Filled out Form I-765
-Copy of your I-485 receipt notice
-Color copies of the front AND back of ALL EAD cards ever issued to you
-Color copy of a government-issued ID (i.e. your passport photo/bio page OR
a copy of the front AND back of a driver's license—we don’t need both)
-Two passport-sized photos (2 x 2 inches) following these guidelines
