1. Form I-765, Application for Employment Authorization
2. No additional fee required if concurrently filed with I-485
application
3. Two photos meeting USCIS requirements (Attached to the form)
4. Copy of I-485 Receipt Notice (if not concurrently filed with I-485)
5. Copy of Form I-94, both sides.
6. Copy of passport biographical page and visa stamps.
7. Copy of I-797 Approval Notice(s) for H1b and OPT, and OPT card both
sides.
Advance Parole Application
1. Form I-131, Application for Travel Document
2. No additional fee required
3. Two photos (Attached to the form)
4. Copy of I-485 Receipt Notice (if not concurrently filed with I-485)
5. Copy of Form I-94, both sides.
16. Copy of passport biographical page and visa stamps.

Four colored copies of biographic and visa pages of current passports as
well as copies of expired U.S. visas for you and your family
• COURT AND PRISON RECORDS (if applicable)
A certified copy of each court record or any prison record must be obtained
for any criminal conviction(s).
• MILITARY RECORDS (if applicable)
Details of any military service – with ranks and period of service.
• PHOTOGRAPHS
Six (6) photographs.
• MARRIAGE DOCUMENTS (if applicable)
Four copies of the marriage certificate; evidence regarding validity of
marriage, including documents in joint names, such as property ownership/
lease, bank accounts and insurance etc; and birth certificate(s) of child(
ren), if any.
• INCOME TAX DOCUMENTATION
Copies of W-2 Statements (if available) and Federal Income Tax Returns from
the last two years.
• FINAL DIVORCE DECREE
Proof of termination of prior marriage(s), such as final decree of divorce,
annulment, or death certificate. Custody documents must be provided for any
minor children who will be applying for permanent residence.

• BIRTH CERTIFICATE

If current or prior H or L, provide four copies of all Forms I-797 (approval
