• Copies of My Ph.D.Degree With U.S. Equivalent Evaluation;
• Copy of I-140 Approval Notice of Exceptional Ability in the
National Interest;
• Copy of My Passport Biographic Data Page;
• Copy of My Current visa and I-94 Card, I-797 Notice of Approvals;
• Proof of All Prior Immigration Status include J-1 Waiver and
DS2019 Form
• 2011 Federal Tax Returns and W-2;
• Current Letter of Employment (With Most Recent Paystub); and
• Filing Fees of $580.00, $1,070 & $1,225.
My spouse, xxx, is applying for derivative status. Her materials (enclosed)
are:
• Form I-485, Application for Adjustment of Status;
• Form G-325, Biographic Data;
• Form I-693, Medical Form;
• Form I-765, Application for Employment Authorization;
• Form I-131, Application for Travel Authorization;
• Six Color Photographs of Aplicant;
• Copy of Birth Certificate (Translated);
• Copy of Marriage Certificate (Translated);
• Copy of My H-1B visa, Current I-94 Card, and I-797 Notice of
Approval;
• Copy of My Passport Biographic Data Page;
• Proof of All Prior Immigration Status;
• Current Letter of Employment (With Most Recent Paystub);and
• Filing fee of $1,070.
PLEASE NOTE: ATTACHED WITH THE INDEX OF SUPPORTING DOCUMENTS IS A COPY OF
THE RECENTLY APPROVED I-140, PETITION FOR IMMIGRANT WORKER, IN THE EB-2
NATIONAL INTEREST WAIVER CATEGORY.
