Filing status:
Are you authorized for Centralized Filing or Sole Jurisdiction or is this
filing for an internationally recognized athlete… ?
应选YES 还是 NO？

Part 1：
Company or Organization Name:
自己DIY,需要填单位名称吗？

Part 3:
Arrival-Departure Record Number(I-94): 是填入境美国的时候用的I-94的号码吧。
在美国境内renew H1B的时候换过I94， 但没离过境。
Travel Document Number: 填了护照的信息以后，是不是这个就不用填了。不知道这一
项是什么文件。

Part 4:
由于不能填中文地址姓名，说是要provide the beneficiary’s foreign address on
a separate piece of paper with your supporting documentation that contains
the applicant’s name, part #, associated question and A#. 这个part# 和A#是
什么？如何操作的？
5)
Part 6:
Is this a permanent position? 薄厚不是永久的，选No应该没问题吧，还是必须选
Yes? 我现在是DIY EB1A。

Part 7: Information on Spouse and All Children of the Person for Whom You
Are Filing
我老婆（H4） Applying for Adjustment of Status? 我现在自己先申请140， 通过后
和老婆一起提交485，此处是否应该选Yes?
