It contains very sensitive questions like "How much you earn in the
last 12 months?", "How many minutes do you need to get to work in
the morning?", "What's your major duty at work?" and "Can you climb
stairs without problem?"
I feel uncomfortable to reply these questions and mail back to them.
However, in the cover letter it stated that it is required by law
that I fill in the survey (28 pages) and return it. I've googled th |

I have to point out the country I was born in and my race (Chinese or
Korean or Filipino, etc.)

how many bath rooms do you have at home and your current and previous
home and work address.)
Basically it can totally be used to do identity theft.

"I had the usual 2 survey mailings and reminder cards. Phone calls started
the second month. I unpluged the phone, leaving the caller ID on. 3rd month
the visits started. Had notes left and a few phone calls from 1 field rep.
Also had a letter from the local CB office introducing my rep. Had a 2nd rep
arrive when the 1st had no luck with me. Phone calls and rep visits ramp up
towards the end. Shortly after the harassment stopped, I received a
